网状蛋白、支架蛋白及转化生长因子β1 mRNA在膀胱出口梗阻性逼尿肌中的表达及意义

摘　　要：目的探讨膀胱出口梗阻（BOO）性逼尿肌中网状蛋白、支架蛋白及转化生长因子β1（TGF-β1）mRNA表达的改变及意义。方法B00组患者16例，为良性前列腺增生（BPH）并经尿动力学压力-流率检查结果证实为高压-低流型；对照组5例，为外伤等情况入院并排除下尿路梗阻病史者。2组患者均行开放手术治疗，同时切取膀胱上壁组织2cm×1cm×1cm，标本行RT—PCR反应，检测膀胱逼尿肌中网状蛋白、支架蛋白及TGF-β1 mRNA表达的改变并比较TGF-β1与网状蛋白、支架蛋白之间的线性相关性。结果BOO组网状蛋白、支架蛋白及TGF-β1，mRNA的表达量分别为（20．0±25．0）×10^6、（25．0±31．0）×10^6、（3．60±7．30）×10^6拷贝数／μg总RNA，对照组分别为（2．2±0．9）×10^6、（2．4±2．1）×10^6、（0．18±0．13）×10^6拷贝数／μg总RNA，2组比较差异均有统计学意义（P＜0．01）。TGF-β1与网状蛋白、支架蛋白之间的直线相关系数r分别为0．952和0．898，P值均＜0．001，呈线性正相关。结论膀胱逼尿肌中网状蛋白、支架蛋白及TGF-β1表达改变均与逼尿肌的功能状态密切相关。Objective To determine the expression of vimentin,desmin and TGF-β1 mRNA in detrusor with bladder outlet obstruction （BOO）. Methods Sixteen patients of BOO were diagnosed with benign prostatic hyperplasia confirmed by urodynamic examination. The control group included S patients who were diagnosed as trauma without evidence of lower urinary tract obstruction. All BOO patients were treated by suprapubic prostatectomy while the 2 cm×1cm×1 cm detrusor biopsies were gained. The expression of vimentin, desmin and TGF-β1 mRNA were determined by RT-PCR. The correlations of TGF-β1 with vimentin, desmin were analized by Pearson correlation method. Results The expression of vimentin, desmin and TGF-β1 mRNA in BOO group were （20.0±25.0） × 10^6, （25.0 ± 31.0） × 10^6 . （3.60±7.30） × 10^6 copies/μg RNA, respectively, and were significantly higher than those of control group （P= 0. 008, 0. 008 and 0. 002）. The linear correlation coefficient ratio of TGF-β1 with vimentin, desmin were 0. 952 and 0. 898. It showed a positive correlation（P〈0. 001）. Conclusions The expression of detrusor vimentin.desmin and TGF-β1 is correlated with bladder detrusor function following BOO.
