摘 要:目的探讨移植肾组织穿孔素(perforin,P)和T细胞内抗原-1(Tcellintracellular antigen-1,TIA-1)的表达及临床意义。方法为查找肾丧失功能的原因取42例移植肾及对照组织标本,采用竞争性PCR技术定量检测P和TIA-1 mRNA水平,通过β-actin对检测结果进行校正,并与组织病理学诊断比较分析。结果急性排斥组、慢性排斥组和无排斥组P的检出例数分别为14、13和7例,TIA-1分别为14、10和3例;急性排斥组P和TIA-1 mRNA水平明显高于慢性排斥组(P<0.05)和无排斥组(P<0.05),慢性排斥组和无排斥组间P的表达亦有显著差异;急性排斥组P和TIA-1 mRNA水平与组织学积分有关;二者表达综合分析判断移植肾急性排斥敏感度为96%,特异度为72%。结论移植肾组织P和TIA-1 mRNA水平的上调与移植肾急性排斥有关,这一结果可能有助于急性排斥的早期预测。Objective To investigate the expressions of perforin ( P) andTcellintracellularantigen-1 (TIA-1) in renal allografts and its clinical significance. Methode Forty-two samples were investigate for diagnosing the etiology of graft dysfunction. The levels of intragraft P and TIA-1 mRNA were quantified by competitive PCR. The results illuminated with the ratio of target gene to β-actin were compared with the histological diagnosis based upon Banff criteria. Results The positive expression ratios of P in acute rejection group, chronic rejection group, and nonrejection group were 14/14, 13/15, and 7/13, respectively, while he positive expression ratios of TIA-1 in the three goups were 14/14, 10115, and 3/13, respectively. Intragraft expressions of P and TIA-1 were significantly heightened in acute rejection group compared with that of chionic rejection group ( P 〈 0.05 ) and nomrekection group ( P 〈 0.05). The P mRNA level in chronic rejection group was higher than that in nonrejection group ( P 〈 0.05). The mRNA levels P and TIA-1 in acute rejection group were correlated with the histologic scores of the sampies, Combined analysis of P and TIA-1 expressions demonstrated a sensitivity of 96% and a specificity of 72% for acute rejection. Upregulafion of the P and TIA-1 mRNA expression in acute rejection samples might serve as markers for early diagnosis of acute rejection of human renal allografts.
