变应性鼻炎的中医辨证分型及其与白细胞介素-4和干扰素-γ的相关性研究

摘　　要：目的研究白细胞介素-4及干扰素-γ与变应性鼻炎(Allergic Rhinitis,AR)中医辨证分型的相关性。方法采用双抗体夹心ABC-ELISA法测定不同证型AR患者外周血血浆白细胞介素-4(IL-4)及干扰素-γ(IFN-γ)水平,对证型和检测值进行相关性分析。结果四型AR患者IL-4水平显著高于正常对照组(P<0.01),IFN-γ水平显著低于正常对照组(P<0.01),IL-4/IFN-γ比值较正常对照组差异有显著性意义(P<0.01)。其中肾阳不足组AR患者IL-4水平最高,其次为脾气虚弱组、肺经伏热组、肺气虚寒组;肾阳不足组AR患者IFN-γ水平最低,其次为脾气虚弱组、肺经伏热组、肺气虚寒组;肾阳不足组IL-4/IFN-γ比值高于正常对照组,其次为脾气虚弱组、肺经伏热组、肺气虚寒组。结论血浆IL-4及IFN-γ水平的改变参与了AR的发病,并与AR辨证分型具有相关性。Objective To investigate the association of IL-4 and INF-γ levels in plasma with patterns differentiation among the cases with allergic rhinitis （AR）. Methods AR patients were differentiated into various patterns based on the principles of TCM theory at first and their plasma levels of IL-4 and INF-γ were measured with the procedures of ABC-enzyme linked immunosorbent assay（ABC-ELISA） ,with a group of healthy people as the normal controlling group. Then analyzed in detail were the correlation of IL-4 and INF-γ levels in peripheral blood with patterns dif- ferentiation data. Results All the AR cases showed a very significantly higher level of IL-4 and a very obviously lower level of INF-γ than that of healthy people （P〈0.01）. The difference in the ratio of IL-4/INF-γ was also very significant between AR cases and healthy people （P 〈0.01 ）. Among the cases with AR, those in the pattern of Kidney Yang deficiency displayed the highest level of IL-4, and decreased higher levels of IL-4 in turn were seen among those patients in the patterns of Spleen Qi deficiency, Lung channel deep-lying heat and Lung Qi deficiency cold respectively. However, the level of INF-γ was the lowest in the cases with the pattern of Kidney Yang deft ciency, and gradually increased levels of INF-γ from the lowest value in turn were seen in the patterns of Spleen Qi deficiency, Lung channel deep-lying heat and Lung Qi deficiency cold respectively. The level distribution of IL-4/INF-γ ratio was similar to that of IL-4, with a same order of association with the patterns as above. Conclusions The changes of IL-4 and INF-γ levels in plasma are associated with pathogenesis of AR and hold some certain correlation with the paltterns differentiation among the cases with allergic rhinitis.
