透明质酸－链霉素圆窗灌注术抗致残性眩晕的效用  

THE EFFECT OF HYALURONAN PERFUSION THROUGH THE ROUND WINDOW ON DISABLING VERTIGO

摘　　要：为探索一种既消除眩晕又保存听力的治疗致残性眩晕的药物性迷路切除术，应用透明质酸－链霉素（ＨＡ－ＳＭ）圆窗灌注术治疗６例梅尼埃病（Ｍｅｎｉｅｒｅ病，ＭＤ）和４例迟发性内淋巴积水（ＤＥＨ）。结果：９例眩晕控制，８例听力不变或提高，７例耳鸣耳闭减轻或消失，前庭功能检测示眼震时间明显缩短（Ｐ＜０００１）。结论：ＨＡ－ＳＭ圆窗灌注术可选择性次全或完全破坏前庭功能，消除眩晕，保存听力，前庭功能代偿快，手术安全有效。To study an effective treatment for preserving the hearing as well as eliminating the disabling vertigo, a pharmacologic labyrinthectomy was investigated Six patients with Meniere disease (MD) and 4 with delayed endolymphatic hydrops (DEH) were treated with hyaluronan streptomycin (HA-SM) perfusion through the round window The results were as follows: 9 patients were free of vertigo attacks; hearing was unchanged or improved in 8 patients; tinnitus and fullness were reduced or disappeared in 7 patients The nystagmus duration induced by the caloric test (ENG) and the rotation test was remarkablely shortened compared with that of the preoperation ( P <0 001) In conclusion, HA-SM perfusion can subtotally or totally destroy vestibular function resulting in the eliminat on of the vertignous spells without impairment of hearing for most patients. The compensation of vestibular function was rapid in all patients
