联合测定孕酮及CA_(125)在预测先兆流产预后的诊断价值

摘　　要：目的:探讨联合测定血清孕酮及CA125在预测先兆流产预后的价值。方法:选择201例孕龄在5～12周的单胎孕妇,根据妊娠结局分为3组,正常妊娠组80例,先兆流产组68例,难免流产组53例,各组无年龄、孕产次及体重差异,测定血清孕酮及CA125水平。结果:安胎成功者孕酮水平高于流产组(P<0.05),CA125低于流产组(P<0.05)。以孕酮<16.3ng/ml为临界值,预测流产敏感性、特异性及准确性为86.2%、83.1%、85.2%,以CA125值>49.1U/ml为临界值,预测难免流产的敏感性、特异性及准确性分别为79.1%、81.2%、82.1%,如同时以孕酮<16.3ng/ml及CA125>49.1U/ml为临界值,预测难免流产的敏感性、特异性及准确性分别为78.3%、86.2%、90.1%。结论:在预测先兆流产预后的检测中,联合测定血清孕酮及CA125比单项测定指标的预测准确率明显提高。Objective： Assess the value of combine serum progesterone and CA125 for forecast the outcome of threatened abortion Methods： Clinical data of 201 cases of early monovular pregnancy （5 - 12 weeks） were analyzed and divided into three groups： normal intrauterine pregnancy group （ NIUP）, threatened abortion group and inevitable abortion group. Serum progesterone and CA125 level were assayed in all women. Results： The progesterone level of inevitable abortion was significantly lower than that of threatened abortion （P 〈0. 05） . On the contrary, the CA125 level of inevitable abortion was significantly higher than that of threatened abortion （P 〈0. 05） . The serum progesterone and CA125 level assay is valuable in detecting inevitable abortion. Separate serum progesterone 〈 16. 3 ng/ml and CA125 〉49. 1 U/ml for the critical value. The sensitivity, specificity and accuracy were 86. 2% , 83. 1%, 85. 2% and 79. 1%, 81.2%, 82. 1%, ff combine serum progesterone 〈 16. 3 ng/ml and CA125 〉 49. 1 U/ml, the three rates would be 78. 3%, 86. 2%, 90. 1%. Conclusion： Combine serum progesterone and CA125 level assay is more valuable than single index in forecast the outcome of threatened abortion.
