摘　　要：目的:探讨骨瓣成形术治疗横窦骑跨性硬膜外血肿的疗效。方法:分析骨瓣成形术治疗的36例横窦骑跨性硬膜外血肿的临床资料:全部病例术前经CT确诊,行骨瓣成形术清除横窦骑跨性硬膜外血肿。单侧血肿<20 mL者采用旁正中切口,单侧血肿≥20 mL者采用枕后马蹄形切口或枕下倒钩形切口,双侧血肿采用正中切口。结果:术后复查CT横窦减压效果良好。按格拉斯哥预后评分术后恢复良好29例,重度残疾2例,中度残疾3例,植物生存1例,死亡1例。结论:骨瓣成形术清除横窦骑跨性硬膜外血肿安全且能更好地避免横窦受压,骨瓣复位符合解剖复位原则,可作为治疗横窦骑跨性硬膜外血肿较理想的方法。Objective： To investigate the effect of craniotomy on treatment of epidural hematoma stradding transverse sinuses. Methods：The clinical data of 36 patients with straddding transverse sinuses epidural hematoma treated by craniotomy were analyzed. Results：The pressure on the sinus was relieved effectively in all cases. Based on GOS, the clinical curative effect of these patients after operation were evaluated：29 cases recovered well,2 cases suffered from moderate disability, 3 cases remained severe disability, 1 suffered from persistent vegetative state and 1 cases died. Conclusion.Craniotomy in treatment of epidural hematoma stradding transverse sinuses has advantages of safe and anatomical replacement, the pressure on the sinus can be relieved effectively. It is a better operative way in treatment of epidural hematoma straddding transverse sinuses.
