后腹腔镜肾蒂淋巴管结扎术治疗乳糜尿(附16例报告)

摘　　要：目的：探讨后腹腔镜肾蒂淋巴管结扎术治疗乳糜尿的手术方法和临床效果。方法：16例患者（男9例，女7例；左侧6例，右侧10例；年龄46～68岁，平均52．1岁）均行后腹腔镜肾蒂淋巴管结扎术。结果：16例患者手术均成功，平均手术时间100min，术中出血量平均70mL，未发现明显的并发症，术后平均住院7．3d。患者出院时尿液均清亮，16例单侧乳糜尿患者尿乙醚试验全部阴性。随访2年至6个月无复发。结论：该术式具有微创、出血少、恢复快等特点，淋巴管结扎更加完全，近期效果良好。Objective： To improve diagnosis and treatment of chyluria,to evaluate the surgical technique and clinical efficacy of retroperitoneoscopic ligation of renal lymphatic vessels for the treatment of chyluria. Methods： Sixteen cases of chyluria （9 males and 7 females, with age ranging from 46 to 68 years） , were selected for retroperitoneoscopic ligation of renal lymphatic vessels. Results： All cases were successfully treated with retroperitoneoscopic management. The average operation time was 100 minutes, the average blood loss was 70 mL and the average hospital stay after operation was 7.3 days. No obvious surgical complications occurred. The urine from all patients was clear when the patients discharged from the hospital. Chyluria disappeared in the 16 unilateral cases. No recurrence of chyluria was found during the following-up of 2 years-6 months. Conclusion：Retroperitoneoscopic ligation of renal lymphatic vessels for the treatment of chyluria is an efficient surgical procedure and has the advantages of less blood loss, minimal invasion, complete lymphatic vessels ligation, short hospital stay and rapid recovery. So far, it is an ideal surgical procedure for treatment of chyluria.
