摘 要:目的探讨川崎病(KD)血小板活化标志物CD62p和CD63的变化及其临床意义。方法应用流式细胞技术检测32例KD患儿和15例正常对照儿的血小板活化标志物CD62P、CD63阳性表达率,同时检测BPC值。结果KD患儿全血CD62P、CD63阳性表达率及BPC值较正常对照儿明显增高;合并冠状动脉异常者CD62P、CD63阳性表达率增高更加显著,其中以CD62P阳性表达率增高最具有统计学意义;CD62P和CD63的阳性表达率经大剂量丙种球蛋白[400mg/(kg·d)×5d]和阿斯匹林治疗后能明显减低,但仍高于正常对照儿。结论血小板活化过程参与了川崎病免疫性血管炎的病理损害过程。大剂量丙球和阿斯匹林治疗能抑制KD患儿血小板膜糖蛋白的高表达,降低血小板的活化程度。因此在川崎病病程中动态检测血小板活化标志物CD62p、CD63阳性表达率和BPC值,对KD的诊断及疗效判定具有重要的临床意义,尤其是检测CD62p阳性表达率,对预测心脏和冠脉异常是否发生更具有价值。Objective To investigate the clinical significance of platelet activation marker CD62p and CD63 variation in Kawasaki disease(KD). Methods The positive expression rates of CD62p and CD63 in KD patients and normal controls were measured by flow cytometry. Results The positive expression rates of CD62P, CD63 and BPC in KD patients were higher than those of the normal controls, and they were more higher in coronary artery abnormalities group, especially the positive expression rates of CD62P, which had highly significant difference; After aspirin and large dose of IVIG[400 mg/(kg · d) × 5 d] treatment, the positive expression rates of CD62p and CD63 were lower than those before treatment. However, those after treatment were still higher than those in the normal controls. Conclusion The platelet activation participates in the pathological course of immunity vasculitis in KD. The treatment of aspirin and large dose of IVIG can inhabit the high expression of CD62P and CD63, and can degrade the degree of the platelet activation. It is valuable in diagnosis and treatment effects judgment to have dynamic observation of the positive expression rates of CD62p, CD63 and the changes of BPC, especially CD62p, which is of high value inprediction of coronary artery abnormalities.
