摘  要:目的:探讨应用外固定器治疗第一掌骨基底部骨折的临床效果。方法:采用自行研制的微型外固定器牵引固定治疗第一掌骨基底部骨折32例,男24例,女8例;年龄17~62岁,平均31、6岁。骨折未累及腕掌关节,则直接于骨折近端固定骨针,Bennett骨折或Rolundo骨折的近端进针点选在大多角骨。结果:术后随访3—18个月,骨折临床愈合时间5~9周,平均6.7周,无骨折移位、固定松动及严重针道感染等并发症发生,关节功能恢复良好。按照TAM系统评定:优24例,良5例,可3例,优良率90、6%(29/32)。结论:外固定器有调整、拆除方便,固定可靠。创伤小,能早期功能康复活动的优点,是治疗第一掌骨基底部骨折的有效方法。Objective: To explore the effect of micro-external fixation device for treating the basilar part fracture of the first metacarpus. Methods: Thirty-two patients ( male 24 and female 8, age ringing from 17 to 62 years with the mean of 31.6 years) with basilar part fracture of the first metacarpus were treated with micro-external fixation device. The pins were fixed in fractured proximate in case of intact carpometacarpal joints. The trapezium bone in the proximate fractured part was choosed as the needing point in Bennett and Rolundo fracture. Results: All patients were followed up from 3 to 18 months. All fractures healed successfully, the mean union time was 6.7 weeks( range from 5 to 9 weeks). According to standard of TMA,24 obtained excellent result,5 good,3 falr,the rate of excellent and good was 90.6% (29/32). No displacement fracture,fixer loosing,severe pin hole infection were found in the study. Conclusion: The micro-external fixation device can obtain satisfactory effect for the basilar part fracture of the first metacarpus. The fixation device has such advantages as convenient adjustment and removal, reliable fixation, little trauma which can allow early functional rehabilitation.
