摘 要:目的 通过定量CT(QCT)测量昆明地区成年人骨密度,探讨昆明地区成年人骨密度的正常值范围及其影响因素。方法 昆明地区20~85岁560例成年人为研究对象,采用Marcom Mx 8000多层螺旋CT测定腰椎松质骨骨密度,按每5岁为1个年龄组分析QCT骨密度与年龄、牛奶饮用、运动、日照、体质指数(BMI)、腰臀比(WHR)、绝经年限和性激素的关系。结果 (1)昆明地区男性及女性峰值骨密度均出现在20~24岁年龄段,男性为(132.44±3.56)mg/cm^3,女性为(138.92±6.04)mg/cm^3,女性高于男性(P〈0.05)。随着增龄,骨密度逐渐下降,骨丢失率增加,年丢失骨量女性为2.08mg/cm^3,男性为1.37mg/cm^3;(2)骨密度与年龄、腰臀比(WHR)、促卯泡素(FSH)、黄体生成素(LH)及绝经年限呈负相关(r分别为-0.74、-0.25、-0.32、-0.34、-0.65,P〈0.01或P〈0.05),与日照时间、运动量、雌激素(E2)呈正相关(r分别为0.18、0.28、0.23,P〈0.01或P〈0.05),除外年龄的影响,骨密度与WHR呈负相关(r=-0.11,P〈0.05),与每日牛奶摄入量呈正相关(r=0.15,P〈0.05);(3)以T≤-2.5为骨质疏松的诊断标准,骨质疏松的检出率为26.1%,当-3.06≤T≥-3.04时,绝经后妇女骨质疏松的检出率为30.0%。结论 建议以T≤-3.05作为昆明地区QCT诊断骨质疏松症的阈值。Objective To study the change of bone mineral density in adult in Kunming city and to analyze the possible related factors. Methods Five hundred and sixty adults from 20 to 85 years old were classified into the age groups of every 5 years old, the trabeculae bone mass was measured by multi-slice spire CT at lumbar spines. The correlations of the QCT bone mineral density (BMD) with age, exercise, exposure to sun, intake of milk, body mass index (BMI), waist-hip ratio (WHR), duration of menopause and sex hormones were analyzed. Results (1) The peak bone mass of adult was seen in 20-24 years old in male and female groups, which was lower in female [(132. 44±3. 56) mg/cm^3] compared with male [(138. 92±6.04) mg/cm^3 ], P〈0.05. Age was the main factor influencing BMD, and the BMD dropped gradually with age increasing. The bone loss happened at a rate of 2.08 mg/cm^3 each year in female and 1.37 mg/cm^3 each year in male. (2)It was found that BMD was negative correlated with WHR (r=-0.11 ,P=0. 01) and positive correlated with intake of milk(r=0. 15,P=0. 04). (3)The osteoporosis positive rate was 26.1% for T≤-2.5 SD in adult and 30% for -3. 06≤T≤-3.04 SD in postmenopausal female. Conclusions The peak bone mass of adults in Kunming are (132.44±3. 56)mg/cm^3 in male and (138.92±6. 04)mg/cm^3 in female, T≤-3.05 is suggested as a cutoff value for osteoporosis diagnosis in Kunming.
