摘 要:提出γ、δ角的定义,观察γ、δ角在正常髋关节超声检查中的变化情况,评价其对髋关节的诊断价值。对67名婴幼儿的双侧髋关节进行了超声波检查。67名受检者中男31例,女36例,最小年龄1月,最大年龄1岁半,临床检查均无髋关节疾患。超声频率为4MHz^7.5MHz。所有髋关节均进行静态超声检查,共获得134张图像。应用Graf方法对134张图像进行α角和β角的测量,然后进行γ角和δ角的测量,记录结果并进行统计处理。将得到α、β、γ、δ角的4组536个数据,采用SPSS11.5统计软件对4组数据进行处理。对α、β、γ、δ角进行相关分析:α角与β角两组有弱相关性。α角与γ角组没有相关性,两角度反映的对象在具体的测量方法上虽然不同,但反映的本质却是相同的。γ角与δ角两组有相关性,两组的回归方程:γ=2.68°+0.58°δ。β角与δ角两组没有相关性,虽然两组描述的同一个对象,但在描述相同对象的变化却是相反的,β角越大,软骨发育越差,δ角越大,软骨发育越好。研究髋臼骨顶的测量对于DDH的诊断很有帮助。γ、δ角可以对髋臼进行测量,γ、δ角与α、β角在超声成像时是两种不同的方法。To define angle-γ and angle-δ, and to evaluate their application in the scanning of hip joint by a clinical and geometrical study on normal hips, the sonographic scannning were undertaken in both left and right hips from 67 chlidren. There were 31 males and 36 females. The age at the examination ranged from 1 month ( 18 month years old. There was no sign of abnormal condition in all the involved hips. An ultrasonic scanning examination, using linear 4 MHz and 7.5 MHz probes, was performed in all the children. Each child was examined and 134 pictures were obtained. The angle-α and angle-β were measured according to Graf classification in all the 134 pictures, and the angle-γ and angle-δ were also measured for the geometrical analysis. The resuets shaved that statistical analysis was conducted using the data including 536 figures from the four groups with the SPSS software. The correlation analyses on theα,β,γ and δ angles were conducted. The angle a was tenuously correlated with the angle ft. The angle a was no correlated with the angle γ ; The angle y was correlated with the angle δ, and the regression equation for the angles γ and δ is as γ =2.68° + 0.58°δ. The angle β was no correlated with the angle δ, these two angular parameters were of the opposite meanings and interpretations in describing the variability of the same object, i. e. , the more the value of angle βwas, the worse the cartilaginous development became while the more the angle δ was, the better the cartilaginous development became. It is very helpful for the diagnosis of developmental dysplasia of the hip (DDH) to study the geometrical measurement of the bony acetabulum. The method for the measurements of angles a and β is essentially different from those of angles y and δ in the procedure of ultra- sonic examination. The measurements of angles y and δcan be applied in the scanning of the hip joint, particularly for the early diagnosis of DDH.
