异位妊娠发病因素分析

摘　　要：目的分析30年来异位妊娠发病率的变化,调查2003年异位妊娠发病相关因素,以期能间接反映该院所属地区发病情况,为进一步临床研究提供资料。方法收集华中科技大学同济医学院附属同济医院1973年、2003年异位妊娠与分娩、妊娠(分娩、引产、人工流产)及妇科住院例数,对异位妊娠发病率的变化进行分析;整理2003年271例异位妊娠临床资料,调查其发病相关因素。结果异位妊娠发病率1973年为0.46%,2003增至6.06%,其发病率有显著性差异(P<0.0001)。相关因素中,≥30岁组、初产妇组异位妊娠发生率较高。发现宫腔手术史、剖宫产术史、前次异位妊娠史、盆腔感染史等因素相关性较强。比较有无避孕措施异位妊娠所占比例,认为IUD等不增加异位妊娠发病几率。分析271例异位妊娠中,急诊占32.10%(87例),危重症占15.13%(41例)。结论异位妊娠严重危害广大青壮年妇女身体健康,影响社会生产力,应重视并加强对异位妊娠的研究。Objective： To analyze the change of the incidence of ectopic pregnancy between 1973 and 2003, and investigate the etiologic factors of ectopic pregnancy to reflect the incidence of ectopic pregnancy in our hospital to provide the information for the further clinic investigation. Methods： The ectopic pregnancy cases , labour cases, pregnancy cases and in patient gynecologic cases in 1973 and 2003 were collected to analyze the change of incidence of ectopic pregnancy. The clinic information of 271 cases of ectopic pregnancy in 2003 were collected to investigate the correlation factors. Results： The incidence of ectopic pregnancy in 1973 was 0. 46% and added up to 6.06% in 2003 （P 〈 0. 000 1 ）. The incidence of ectopic pregnancy of women whose age ≥30 and first labour was higher. The closer correlation factors were operation history of uterus cavity, cesarean, former ectopic pregnancy , the infection of pelvis. Compared the contraception measure, IUD did not add the incidence of ectopic pregnancy. There were 87 cases （32. 10% ） of emergency and 41danger cases （ 15.13% ） in 271 cases. Conclusion： Ectopic pregnancy harm the healthy of young women seriousily. It is neceserray to strengthen the investigation of ectopic pregnancy.
