摘 要:目的探讨二氧化碳气腹对糖尿病兔在气腹前后血糖变化的影响及其机制。方法利用四氧嘧啶制作糖尿病兔模型,建立不同压力气腹,观察术前、术后0、2、6、12h各时间点血糖、胰岛素、C肽浓度的变化。结果气腹结束后,血糖、胰岛素、C肽浓度上升,至气腹后12h,10mmHg气腹组血糖、胰岛素、C肽浓度较气腹前无明显差异,15mmHg气腹组血糖、胰岛素较气腹前高。10mmHg和15mmHg气腹组之间,血糖、胰岛素变化无明显差异(P>0.05),C肽浓度变化有明显差异(P<0.05)。结论CO2气腹对糖尿病兔的血糖有明显的影响,且影响程度与气腹压力相关,其机制与气腹引起的应激反应和胰腺损害有关。Objective To investigate the effects of CO2 pneumoperitoneum on blood glucose in diabetic rabbits. Methods Japan white rabbits were made diabetic model with Allxon. CO2 pneumoperitoneum of different pressures were established by closed method. Artery blood samples were collected for measuring blood glouse, and concentrations of insulin, C - peptid by radioimmunoassay,which were repeated before the onset of CO2 pneumoperitoneum 0,2,6,12 hours after deflation. Results After abdominal deflation, there were significant increae in blood glouse, insulin, C - peptid, Until 12 hours after abdominal deflation, the level of blood glouse and insulin returned to formal level in group T10, but did not returned in group T15. There were statistically significant differences between group Tl0 and T15 in C - peptid and no statistically significant differences in blood glouse and insulin. Conclusions Under diabetic conditions,CO2 pneumoperitoneum has an certain influence on blood glouse of the rabbits. The mechanism is concerned with alarm reaction and pancreatic damage induced by CO2 pneumoperitoneum. The degree of injure is correlated with the pressure of pneumoperitoneum.
