摘 要:目的了解外周血铜锌钙镁铁水平与小儿疾病状态之间的相关性。方法341例非健康状态小儿分为高危早产儿(n=55)、高危足月新生儿(n=93)、因病住院小儿(n=54)和儿保门诊中体质欠佳小儿(n=139),采集外周静脉血40ul、应用原子吸收法检测血铜锌钙镁铁水平并进行比较。结果高危早产儿、高危足月儿、普通住院非新生儿和儿保门诊患儿发生缺锌的百分率为44.5%、37.6%、27.8%、18%(递减趋势)、发生缺钙的百分率为67.3%、49.5%、11.1%、3.6%(递减趋势)、发生缺铁的百分率为9.1%、14.0%、59.3%、39.6%(递增趋势);有20.1%的儿保门诊就诊的患儿血钙水平超过正常值的高限。高危早产儿、高危足月儿、普通住院非新生儿和儿保门诊患儿平均血锌值为(65.45±18.66)、(75.60±30.57)、(90.07±34.36)、(107.83±36.20)umol/L,血钙值为(1.24±0.27)、(1.54±0.39)、(1.99±0.54)、(1.94±0.36)mmol/L,血铁值为(10.39±1.61)、(10.48±2.51)、(7.35±1.45)、(7.93±1.67)mmol/L,与高危早产儿相比普通住院小儿和儿保门诊患儿血锌、钙、铁值具有显著性差异(P<0.05)。各组小儿血铜、血镁的水平均保持在正常范围。结论外周血锌钙铁水平与小儿疾病状态之间存在明显的相关性,微量元素缺乏的现象应引起儿童保健工作者的高度重视。Objectives To find out the correlation of blood trace elements copper, zinc, calcium and iron levels with the abnormal health status among children. Methods Three hundred and forty - one children with four types of abnormal health conditions including: high - risk preterm neonates (HRP) (n = 55), high - risk term neonates (HRT) (n = 93 ), hospital- ized children (HC) (n = 54) and the unhealthy children from the health - care out - patient - clinic (OPD) (n = 139). Copper, zinc, calcium, magnesium and iron levels were detected by atomic absorption method from 40ul peripheral blood samples. Results The percentage of zinc deficiency among the HRP, HPT, HC and OPD groups, was 44.5 %, 37.6 %, 27.8 %, and 18 %, the percentage of calcium deficiency of these groups was 67.3 %, 49..S %, 11. 1%, and 3.6 %, and both of calcium and zinc deficiencies showed decrement trends; but the percentage of iron deficiency was 9.1%, 14.0%, 5.9.3%, and 39.6%, and showed an increment trend. In the OPD group, hypocalcaemia was found in 20.1% of the children. In the HRP, HRT, HC and OPD groups, the average blood zinc level was(65.45±18.66), (75.60 ± 30.57), (90.07 ± 34.36), and (107.83±36.20) umol/L, the average calcium level was(1. 24 ± 0.27), (1.54 ± 0.39), ( 1. 99 ± 0.54), and (1.94 ± 0.36) mmol/L, and the average iron level was(10.39 ± 1.61), (10.48 ± 2.51), (7.35 ± 1.45), and (7.93 ± 1.67) mmol/L repectively. The blood zinc, calcium and iron levels of both HC and OPD groups were significantly higher than those of HRP group ( P〈 0.05). Blood copper and magnesium levels were normal among all the groups. Conclusions Blood zinc, calcium and iron levels are significantly correlated to the abnormal status among children. Trace element deficiency should be paid great attention by children health - care clinicians.
