摘　　要：目的:介绍开展地高辛血药浓度监测与临床药学服务的工作模式。方法:解放军白求恩国际和平医院的药师深入临床,以药动学、药效学原理为基础,密切观察病人的病理、生理状态,重视药物间相互作用,制定个体化给药方案。结果:实现了药学监护下的个体化给药,有效地预防了地高辛不良反应,使本院地高辛中毒发生率由1997年的11.24%下降至2005年的5.26%。结论:只有将地高辛血药浓度监测与临床药学服务紧密结合,使地高辛给药方案由常规化、经验化转向个体化,才能真正做到合理用药。Objective： To introduce the work pattern of clinical pharmaceutical care associated with monitoring drug concentration of digoxin. Methods.. Pharmacists in Bethune International Peace Hospital of PLA observed the pathological and physiological changes of patients, and paid much attention to drug interactions, and made the drug therapy regimen on the basis of pharmacokinetic and pharmacodynamic principles. Results.. The adverse reactions of digoxin were markedly prevented and the incidence of digoxin poisoning was decreased from 11. 24% in 1997 to 5.26% in 2005. Conclusion： Only by integrating blood drug concentration monitoring with clinical pharmaceutical care by pharmacists can rational administration of digoxin to patients be realized.
