摘  要:目的研究糖尿病创面愈合过程中炎症细胞数量和粒-巨噬细胞集落刺激因子(GM-CSF)表达的变化,探讨GM-CSF在糖尿病创面延迟愈合中的作用。方法70只C57BL/6小鼠分为野生小鼠组(对照组,n=35)和糖尿病模型组(DM组,n=35)。腹腔麻醉后在背部中线两侧各制作0.8 cm×0.8 cm创面。创面动态摄像并于相应时间段取标本,观察创面组织愈合情况,同时计算创面愈合率;免疫组化染色结合计算机图像分析计数创面中性粒细胞和巨噬细胞;ELISA法测定创面GM-CSF表达。结果创面形成后第3天起,DM组小鼠创面愈合率较对照组明显下降,以创面形成后7 d内变化最为明显;DM组中性粒细胞和巨噬细胞浸润情况,在创面愈合早期显著少于对照组,而在后期则较对照组略为增多;创面形成后第1天,两组小鼠创面GM-CSF表达均明显增高;创面形成后第1天和第3天,对照组小鼠创面GM-CSF表达显著高于DM组。结论炎症细胞浸润延迟可能是糖尿病小鼠创面愈合率低的重要原因;而GM-CSF的低表达则可能与创面愈合早期炎症细胞浸润减少有关。Objective To study the infiltration of inflammatory cells and expression of granuloeyte-macrophage colony-stimulating factor (GM-CSF) during the wound healing in diabetic mice and to analyze the mechanisms of GM-CSF in wound healing in diabetes mellitus (DM). Methods Seventy C57BL/6 mice were obtained and divided into control group (n = 35) and DM group (n = 35). All mice were received full thickness skin wound (0.8 cm ×0. 8 cm) in each side of midline after deep anesthesia. The wound sites were digitally photographed to calculate the percentage of wound closure using computer image analysis software in different time points post-injury. The wound specimens were also obtained dynamically for detecting the infiltration of polymorphonuclear neutrophilics ( PMN ) and macrophages (Mφ) using immunohistology technique. The content of GM-CSF in the wound sample was measured by ELISA. Results The analysis of wound closure showed that wound healing in DM mice was significantly delayed compared with that in the control mice from the third day post wound. In the DM mice, the PMN and Mφ infiltrations to wound sites were reduced significantly in the early phase during the wound healing, while remained a little higher than those in the control group in the later phase. The levels of GM-CSF expression were increased markedly from the first day post wound and declined to lower levels in the two groups. At the first day and third day post wound, GM-CSF levels in DM mice were significantly lower than those in the control mice. Conclusion The delayed wound closure in DM mice may be due to failure of infiltrations of inflammatory ceils and lower expression of GM-CSF in wound site.
