摘 要:目的探讨外周血 CD_4^+CD_(25)^+调节性 T 淋巴细胞(CD_4^+CD_(25)^+Tr 细胞)的比例和功能在原因不明复发性流产(URSA)中的作用。方法采用双荧光标记流式细胞分析技术检测15例 URSA 患者(流产组)、15例正常非妊娠妇女(正常非孕组)和13例正常妊娠妇女(正常妊娠组)外周血中CD_4^+CD_(25)^+Tr 细胞的比例。采用磁珠分选方法从3组妇女外周血淋巴细胞中分离 CD_4^+CD_(25)^+Tr 细胞和CD_4^+CD_(25)^-T 淋巴细胞,通过体外增殖抑制实验计算抑制率来评价 CD_4^+CD_(25)^+Tr 细胞对 CD_4^+CD_(25)^-T 淋巴细胞的抑制功能。结果流产组妇女外周血中 CD_4^+CD_(25)^+Tr 细胞的比例为(6.9±1.8)%,明显低于正常非孕组的(10.8±1.1)%(P<0.05)和正常妊娠组的(11.2±1.4)%(P<0.01)。流产组妇女CD_4^+CD_(25)^+Tr 细胞的抑制率为(75±6)%,明显低于正常非孕组的(89±4)%(P<0.05)和正常妊娠组的(90±4)%(P<0.01)。正常非孕组与正常妊娠组妇女 CD_4^+CD_(25)^+Tr 细胞的比例和抑制率比较,差异均无统计学意义(P>0.05)。结论 URSA 的发生可能与外周血 CD_4^+CD_(25)^+Tr 细胞的比例下降和功能降低有关。Objective To investigate the proportion and function of CD4^+ CD25^+ regulatory T cells (CD4^+ CD25^+ Tr) in unexplained recurrent spontaneous abortion (URSA). Methods (1) Proportion measurement: the proportion of CD4^+ CD25^+ Tr cells in peripheral blood was measured by double-label flow cytometric analysis. The samples were taken from 15 URSA women, 15 normal non-pregnancy women and 13 normal pregnancy women. (2) Function measurement: CD4^+ CD25^+Tr cells and CD4^+ CD25^- T cells were extracted from peripheral blood lymphocytes by the microbeads separation. The purity of CD4^+ CD25^+ Tr cells and CD4^+ CD25^-T cells was measured by flow cytometry. The growth inhibitory effect of CD4^+ CD25^+ Tr cells on CD4^+ CD25^-T cells was assessed in vitro. Results The proportion ofCD4^+ CD25^+ Tr cells was decreased significantly in URSA women ( 6. 9 ± 1.8 ) % than that in normal non-pregnancy women [ ( 10. 8 ± 1.1 ) % ] (P 〈 0. 05 ) and normal pregnancy women [ ( 11.2 ± 1.4 ) % ] ( P 〈 0. 01 ). Moreover, the suppressive rate of CD4^+ CD25^+ Tr cells was decreased significantly in URSA women ( 75 ± 6 ) % than that in normal non- pregnancy women [ ( 89 ± 4 ) % ] ( P 〈 0. 05 ) and normal pregnancy women [ ( 90 ± 4 ) % ] ( P 〈 0. 01 ). However, with respect to the proportion and function of CD4^+ CD25^+ Tr cells, there was no significant difference between normal non-pregnancy women and normal pregnancy women ( P 〉 0. 05 ). Conclusion The results suggest that decrease in proportion and function of CD4^+ CD25^+ Tr cells may be associated with URSA,
