中西医结合治疗遗传性出血性毛细血管扩张症致鼻出血疗效观察

摘　　要：目的:探讨遗传性出血性毛细血管扩张症所引起鼻出血的治疗方法。方法:根据遗传性出血性毛细血管扩张症的特点,鼻腔内有毛细血管扩张,引起鼻腔的反复出血,前鼻孔填塞压迫止血无效,采用微波热凝的方法,微波输出功率30-40W,探头接触鼻腔内出血区及毛细血管扩张处,黏膜组织局部变白,输出时间1-2 s。辨证:气阴两虚。治则:益气养阴止血。方用玉女煎加减。结果:患者经过3次鼻腔内微波热凝治疗和中药口服后,基本控制了鼻腔出血,患者自觉无头昏、倦怠乏力、心跳气短的情况,患者鼻腔内不能窥见明显毛细血管扩张及出血点,鼻腔黏膜光滑,随访6个月,未复发。结论:微波治疗该病引起的鼻出血是对症治疗的方法之一,与中药的全身治疗合用,收到既迅速局部止血,又从全身调理用药,全身的影响小,恢复快,达到标本兼治的目的。Objective：To study the treatment of nosebleed caused by hereditary hemorrhagic telangiectasia （HHT）. Methods： According to the characteristic of hereditary hemorrhagic telangiectasia,telangieetasia in nasal cavity causes the repeating nosebleed. It is ineffective to stop bleeding by ramming and pressing the anterior nares, so I take microwave curdling,with the microwave output power being 30 -40W. When the probe touches the bleeding area in the nasal cavity and the area of telangiectasia, mucous membrane tissue becomes partly pale, and the touch time is 1 - 2 second. Differentiation of symptoms and signs ：deficiency of both vital energy and Yin. Through treatment , supplement the vital energy can be achieved as well as nourishing Yin and stopping bleeding. The prescription is to use more or less Yunujian. Results：After three times of microwave curdling in the nasal cavity and the take of Chinese medicine, nosebleed is stopped basically, and the patient feel no dizziness,lassitude, acratia and no palpitation and shortness of breath. There is no obvious telangiectasia and bleeding part in the nasal cavity, mucous membrane of the nasal cavity is smooth. During the following six months visit, the symptome does not recur. Conclusion：Microwave is one of the treatment of nosebleed caused by hereditary hemorrhagic telangieetasia. If we combine it with the Chinese medicine, we can not only stop bleeding partly, but we can also use medicine wholly,thus the effect on the whole body is little,and the patient will recover soon,consequently we can treat both the Drincipal and the secondary aspects of a disease at the same time.
