婴幼儿和成人体外循环心内直视术后并发症与死亡率的比较分析

摘　　要：目的:总结191例婴幼儿和成人体外循环心内直视术后并发症、死亡原因和年龄分布特点。方法采用体外循环心内直视手术治疗各种先天性心脏病患者,其中婴幼儿52例(男24例,女28例),年龄1月～36月,平均11.7±8.3月;成人139例(男72例,女67例),年龄24～55岁,平均41.2±14.5岁。结果52例婴幼儿术后并发症11例,占21.15%;死亡7例,占13.46%;以1岁内的婴幼儿死亡率为最高(占19.05%)。139例成人术后并发症13例,占9.35%;死亡2例,占1.44%;婴幼儿与成人术后并发症和死亡率比较均有显著性差异(p<0.05)。结论采用体外循环心内直视手术的患者年龄越小死亡率越高,同时运用体外循环心内直视手术治疗婴幼儿各种先天性心脏病时应逐步完善心肺的保护措施。Objective： To summarize 191 cases of the concurrent syndrome and death rate after the infant and adult extracorporeal circulation direct-view inner heart operation. Method： To treat all kinds of congenital heart disease by extracorporeal circulation direct-view inner heart operation, among which, 52 cases for infants （24 for male and 28 for female）, age from 1 month to 36 months, the average age is 11 .7 ± 8.3 months, 139 cases of adults （72 for male and 67 for female）, age from 24 to 55, the average age is 41.2 ±14.5 years old. Result： 11 cases of the 55 infants got concurrent syndrome after the operation, the affection rate is 21.15%; 7 cases of death, the death rate is 13.46%; the highest rate exist in infant which is less than 1 year old （the death rate is 19.05%）. 13 of the 139 cases of adult got concurrent syndrome ,and the affection rate is 9.35%, 2 cases of death, the death rate is 1.44%, there are evident difference after the comparison of infant and adult after operation concurrent syndrome and death rate （p 〈0.05）. Conclusion： The younger the patient is, the higher rate of death after extracorporeal circulation direct-view inner heart operation, at the same time protect measures should be take to the lung and heart after extracorporeal circulation direct-view inner heart operation.
