摘  要:目的测定微型染色体维持蛋白2(MCM2)、诱生型环氧化酶(COX-2)及p53在肺鳞癌病理组织和正常肺组织中的表达,探讨该表达与肺鳞癌临床病理特征及预后的关系。方法采用免疫组化法测定81例肺鳞癌病理组织和12例正常肺黏膜组织中MCM-2、COX-2及p53表达,对81例术后的患者随访8~60个月。结果81例肺鳞癌中MCM2的表达越高,肿瘤分化程度越低,TNM分期越高(P<0.001),有淋巴结转移者MCM2的表达高于无淋巴结转移者;但是,COX-2表达越高,肿瘤分化程度越高,TNM分期越低,有淋巴结转移者低于无淋巴结转移者(P=0.042);有淋巴结转移者p53的表达高于无淋巴结转移者(P=0.049);MCM2、COX-2和p53三者两两之间无相关性。单因素生存率分析提示MCM2高表达者的累积生存率明显低于低表达者,COX-2和p53的表达与疾病的预后无关。多因素Cox模型筛选出肿瘤的TNM分期、淋巴结转移状况和MCM2是影响疾病预后的独立因素。结论检测MCM2、COX- 2及p53在肺鳞癌中的表达水平对判断肺鳞癌的恶性程度及预后提供了参考依据。Objective To detect the expressions of minichromosome maintenance protein 2 (MCM- 2), cyclooxygenase-2 (COX-2) and p53 in lung squamous cell carcinoma in order to clarify their clinical features and prognostic significance. Methods EliVision^TM plus immunohistochemical method was used to detect the expressions of MCM2, COX-2 and p53 in 81 cases of lung squamous cell carcinoma and 12 cases of normal lung tissues, all patients were followed up for 8-60 months. Results A significantly higher MCM2, COX-2 and p53 positive expression were observed in the lung squamous cell carcinomas than in the normal lung tissues (P 〈0.01). The positive expression of MCM2, COX-2 and p53 were correlated with tumor differentiation, lymph-node metastasis and TNM stages, but no relationship was found among these three factors. Log-rank test showed that patients with higher positive expression of MCM2 had poor prognosis, however, no relationship between prognosis and COX-2 or p53 was found. The analysis of Cox model showed that TNM stages, lymph node metastasis and positive expression of MCM2 were independent prognostic factors for patients with lung squamous cell carcinoma. Conclusion MCM2, COX-2 and p53 play different roles in oncogenesis and progress of lung squamous cell carcinoma. Detection of them may be useful for the evaluation of the progress and prognosis of lung squamous cell carcinoma.
