摘 要:目的观察乌体林斯(简称U)对哮喘病儿外周血Th1/Th2功能平衡的影响。方法分离20例哮喘发作期病儿和15例健康儿童外周血单个核细胞(PBMC),分别经植物血凝素(PHA组)、PHA+U1(草分支杆菌混悬物浓度为1×10-2g/L,U1组)、PHA+U2(草分支杆菌混悬物浓度为1×10-3g/L,U2组)刺激后,对培养上清液中白细胞介素4(IL-4)和γ-干扰素(IFN-γ)含量进行测定。结果哮喘病儿PBMC经PHA诱生后IFN-γ水平及IFN-γ/IL-4比值明显低于健康儿童组(t=3.05、4.16,P均<0.01);而IL-4水平则明显升高(t′=11.18,P<0.01)。哮喘病儿U1组PBMC诱生IFN-γ、IL-4水平及IFN-γ/IL-4比值明显高于单纯PHA组(F=33.69~262.63,q=7.34~31.76,P均<0.01),也明显高于U2组(q=4.96~21.41,P均<0.01);哮喘病儿U2组PBMC诱生IFN-γ水平及IFN-γ/IL-4比值明显高于单纯PHA组(q=10.35、7.38,P均<0.01),而IL-4水平略低(q=3.29,P<0.05)。结论乌体林斯(1×10-2g/L)可明显提高哮喘病儿PBMC诱生IFN-γ和IL-4水平,尤以前者为著,在促进Th细胞成熟的同时上调Th1功能,改善哮喘病儿Th1/Th2功能失衡状态。Objective To study the effects of Utilins on functional states of Th1/Th2 in peripheral blood mononuclear cells (PBMC) of children with asthma. Methods Twenty sick children and 15 healthy controls were studied. After stimulation to PBMC of asthmatic children and of healthy children withPHA,PHA+U1 (1×10^-2g/L),PHA+U2 (1×10^-3 g/L), respec tively, and cultured for 48 hours in vitro, the levels of and IL-4 in the supernatants of PBMC were detected by ELISA. Resuits After stimulation with PHA,the level of IFN-γ produced by PBMC of the sick children were significantly lower than that of the healthy children's PHA group (t= 3.05, P〈0.01), but the level of 11.4 produced by PBMC of asthmatic children were higher than that of the healthy controls (t'= 11.18,P〈0.01). The ratio of IFN-γ/IL-4 of asthmatic children was lower than that of the healthy children's PHA controls (t= 4.16,P〈0.05). The levels of IFN-γ , IL-4 and the ratio of IFN-γ/IL-4 of U1 group of asthmatic children increased compared to those of the PHA group (F=33. 69-262.63,q=7.34-31. 76, average P〈0.01). The levels of IFN-γ , IL-4 and the ratio of IFN-γ/IL-4 of U1 group of asthmatic children increased compared to those of the U2 group (q=4. 96-21. 41, P〈0.01). After stimulation with PHA+U2, the levels of IFN-γ and the ratio of IFN-γ/IL-4 in PBMC of asthmatic children were significantly higher than those of the PHA group (q= 10. 35,7.38 ; P〈0.01), but the levels of IL-4 were lower than those of the PHA group (q=3.29,P〈0.05). Conclusion Utilins (1 × 10^-2 g/L) can significantly increase the levels of IFN-γ and IL-4 in asthmatic children, and can further improve the imbalance of Tb1/Tb2 function of asthmatic children by inducing the production of Th1 type cytokine (IFN-γ) markedly.
