椎弓根钉加椎间植骨治疗腰椎滑脱症

摘　　要：目的评价椎弓钉系统加椎间植骨治疗腰椎滑脱的疗效。方法对1999年2月～2005年1月我院收治的58例腰椎滑脱患者椎弓根螺钉系统作滑脱复位后椎间植骨治疗。随访时观察患者术前、术后的JOA评分,滑脱率,滑脱角,复位率,椎间隙高度,椎间孔高度。结果全部病例随访1年6个月～7年,平均2年6个月。疗效根据Nakai评分标准优良率达80%。9例Ⅰ度滑脱患者术后全部解剖复位;45例Ⅱ度滑脱患者5例留有Ⅰ滑脱;4例Ⅲ度滑脱患者1例留有Ⅰ度滑脱。术后与术前比较,滑脱率,滑脱角,复位率,椎间隙高度,椎间孔高度有明显改善(P<0.01)。结论椎弓钉系统和椎间植骨治疗腰椎滑脱,疗效肯定。Objective To evaluate effect of pedicle screw system and lumbar interbody fusion in lumbar spondylolisthesis.Method 58 consecutive patients with isthmic spondylolisthesis were restrospected from February 1999 to January 2005,they were treated surgically with pedicle screw system and lumbar interbody fusion. The JOA scales, slipping percentage, reduction percentage, slipping angle, heights of intervertebral space and fusion rate of preoperation, pstoperation were observed respectively.Results The follow-up period was 18-24 months, averaged 30 months. The excellent and good rates were 80 percent, according to Nakai criterion.9 patients with one degree spondylolisthesis were all reduced anatomically atter surgery;45 patients with two degree spondylolisthesis were reduced anatomically except for 5 patients with one degree residual spondylohsthesis;4 patients with three degree spondylolisthesis were reduced anatomically except for 1 patients with one degree residual spondylolisthesis.there were great changes in slipping percentage, reduction percentage, slipping angle, eights of intervertebral space and fusion rate between preoperation and postoperation（P〈0.01）.Conclusion Pedicle screw system and lumbar interbody fusion in isthmic spondylolisthesis has a good results.
