摘 要:目的探讨良性前列腺增生(benign prostatic hyperplasia,BPH)的超声造影峰值强度与血管生成的相关性。方法采用对比脉冲序列(CPS)技术及ACQ(Axius^T auto tracking contrast quantification)定量分析软件,对48例BPH患者进行超声造影检查并记录内腺及外腺超声造影的峰值强度;免疫组织化学方法分析48倒BPH内腺及外腺组织穿刺标本及9例尸检BPH标本中内腺及外腺微血管密度(microvessel density,MVD),比较内腺及外腺超声造影峰值强度与MVD之间的相关性。结果48例BPH患者的内腺及外腺超声造影峰值强度平均为(90.45±42.19)dB及(65.32±25.15)dB,内腺显著高于外腺(t=11.032,P=0.000);内腺及外腺MVD值平均为29.44±11.56及19.11±5.88,内腺显著高于外腺(t=4.429,P=0.002);BPH内腺及外腺造影峰值强度与MVD之间存在着显著的相关性(r=0.838及r=0.876,P=0.000)。尸检BPH标本中内腺及外腺的MVD值分别为28.1±7.95及17.8±8.3,内腺显著高于外腺(t=11.093,P=0.000)。结论BPH超声造影峰值强度与组织内的血管生成存在显著的相关性,可在宏观上显示BPH的血管生成情况。Objective To study the peak intensity in contrast ultrasonography of benign prostatic hyperplasia(BPH) and its relationship with angiogenesis and to provide a woundless index for angiogenesis evaluation in clinic. Methods Contrast ultrasonography was done in 48 BPH patients and ACQ(Axius^TM auto tracking contrast quantification ) technique was used to analyze the peak intensity. Immunohistochemistry was performed to evaluate the level of microvessel density(MVD) in the inner gland and outer gland of BPH biopsy tissues and 9 cases of autopsy tissues. Then the relationship between peak intensity and the level of MVD was analyzed. Results The mean peak intensity of prostatic inner gland and outer gland of biopsy tissues were (90.45 ± 42. 19)dB and (65.32 ± 25.15)dB, respectively. The MVD of inner gland and outer gland biopsy tissues were 29. 44 ± 11.56 and 19. 11 ± 5.88, respectively. The MVD of inner gland and outer gland of autopsy tissues were 28. 1 ± 7.95 and 17.8 ± 8.3, respectively. The mean peak intensity and MVD of inner gland were significantly higher than those of outer gland. The mean peak intensity was significantly correlated to the level of MVD in BPH tissues( r = 0. 838 and r = 0. 876, P = 0. 000). Conclusions The peak intensity of BPH in contrast ultrasonography has significant relationship with angiogenesis and it might be a woundless index for angiogenesis evaluation in clinic.
