正位尿道口阴囊纵隔带蒂皮管一期修复尿道下裂（附24例报告）

摘　　要：为了提高尿道下裂手术的成功率，降低并发症的发生，为了满足患者的心理，使外观更趋于完美的要求，近８年来我们采用正位尿道口阴囊纵隔皮管一期修复尿道下裂２４例，除２例并发尿道口狭窄，余２２例一次成功，成功率达９２％。体会到阴茎头隧道用竹针从阴茎腹侧白膜和阴茎头白膜间向阴茎头尖端刺入不穿透皮肤作支撑，用电刀沿竹针（绝缘体）切割阴茎头海绵体能减少并发症，提高成功率。同时对带有血运的带蒂阴囊纵隔皮管成形尿道和术前、术中、术后的注意事项作了讨论。另外。For raising success rate of hypospadias repair and reducing its complications ,as well as to meet the cosmetic needs in the patients ,we performed 24 NAGPI1-stage repair of hypospadias with scrotal septal vascular pedicle skin tube during the last 8 years .Successful result has been achieved in 22(92%) while urethral stricture was compilcated in the other two.According to our experience, the modified glans tunneling procedure ,in which a tunneling bamboo stick as a sustentaculum was pierced into glans at the junction of ventral tunica albuginia between penis body and glans and advanced to ,but not penetrated through,the glans tip then some of corpus spongiosum around the bamboo stick was eledtron-resected ,coudl reduce the compliations in hypospadias repair .Our experience in urethroplasty with scrotal septal vascular pedicle skin tube and its pre-,inrta-and post-operative points for attention were pressented .In addition,the glans tunneling rather than glans splitting procedure was highly recomended in MAGPI hypospadias repair.
