急性带状疱疹三种治疗方法疗效的对比观察

摘　　要：６０例胸部带状疱疹患者分为三组，每组２０例，各组年龄、性别、身体质量指数等一般情况均无统计学差异性。Ｉ组接受皮肤科住院综合治疗，Ⅱ组、Ⅲ组为门诊病人，分别接受硬膜外注药治疗和胸椎旁神经根注药治疗，每隔３～５天注射一次，共１～３次。使用疼痛缓解视觉模拟评分法（ＶＡＳ）评估疼痛缓解程度。结果表明，Ｉ组接受多种药物及光疗等综合治疗，止痛优、良率仅达３０％，而Ⅱ、Ⅲ组均为１００％。Ｉ组平均病程为１６．５天，而Ⅱ、Ⅲ组分别为１１．４和１２．１天，Ⅱ、Ⅲ组与Ｉ组相比较，差异具有统计学显著性（Ｐ值分别＜０．０５）。以上对比观察结果提示：硬膜外和椎旁神经根注药止痛效果好，病人康复快。Acute herpes zoster(AHZ),a painful virus disease,occurs in patients with defective immune function.This paper reports the treatment of 60 AHZ patients in three groups,group Ⅰ:combined therapy, including antiviral drugs, vitamins,analgesics and physicaltherapy; group Ⅱ:epidural analgesia with 1% lidocaine,vitamine B1100mg, vitamine B12 0.5mg and dexamethasone 3～5mg,at a volume of 2～3ml for every segment; each patient received 1～3 treatments at an interval of 3～5 days. Group Ⅲ:paravertebral nerve root blockwith the same prescription as for epidural injection.Compared with groupI,groupⅡ and Ⅲgot significantly better therapeutic effect.The percentage of excellent and good were both100% in group Ⅱ and Ⅲ, but only 30% in group I.The average course of disease was 16.5 days in group I and was only 11. 4 and 12.1 days in group Ⅱ and Ⅲ respectively.The differences between group I and group Ⅱ /Ⅲ were significant (P<0.05).No serious side effects were observed in the cases being treated. The author stressed the importance of correctperformance of the injection procedures and an observation period of 20 to 60 min after theinjection for the outpatient.
