降糖合剂配合耳针治疗2型糖尿病胰岛素抵抗患者40例临床观察

摘　　要：目的研究降糖合剂配合耳针治疗2型糖尿病胰岛素抵抗的临床疗效及其作用机理。方法将120例2型糖尿病胰岛素抵抗患者按照分层随机的方法,以1∶1∶1的比例分别进入西药(二甲双胍)组、中药(降糖合剂)组和中药加耳针组。疗程均为1个月,观察临床疗效及治疗前后空腹血糖(FPG)、空腹胰岛素(FINS)、餐后2小时血糖(P2hPG)、糖基化血红蛋白(HbA1c)、血脂、胰岛素敏感指数(ISI)、胰岛素抵抗指数(IR)的变化。结果中药加耳针组总有效率92.5%,明显优于中药组(P<0.05)与西药组(P<0.01);治疗前后症状体征改变、中医证候积分、血糖、HbA1c、血脂、FINS、ISI、IR等方面,中药加耳针组与其他两组比较,差异均有显著性意义(P<0.05或P<0.01),并且无不良反应。结论中药降糖合剂配合耳针治疗2型糖尿病疗效显著,且能明显改善患者的胰岛素抵抗。Objective To study the therapeutic effect and mechanism of Jiangtang Mixture combined with ear acupuncture for type 2 diabetic insulin resistance（IR）. Methods Totally 120 cases of type 2 diabetic IR were randomly and equally divided into western medicine （Metformin） group ,Chinese medicine （Jiangtang Mixture） group and Chinese medicine with ear acupuncture group. The treat- ment lasted for one month. The therapeutic effect, fasting plasma glucose （FPG）, fasting insulin （FINS）, postprandial 2 hours plasma glucose （P2hPG）, hemoglobin A lc （HbAlc）, blood fat, insulin Sensitivity index （ISI） and insulin resistance index （IRI） were observed before and after treatment. Results The total effective rate in Chinese medicine with ear acupuncture group was 92.5% ,which was superior to those in Chinese medicine group （P〈0.05） and western medicine group （P〈0.01）. There were significantly differences in symptoms, signs,pattern integrals,blood glucose,HbAlc blood fat ,FINS, ISI and IRI between Chinese medicine with ear acupuncture group and other two groups after treatment（P〈0. 05 or P〈0. 01）. Conclusion Jiangtang Mixture combined with ear acupuncture can relieve IR obviously and cure type 2 diabetic IR effectively.
