妊娠期肝内胆汁瘀积症血脂变化及与母儿预后关系分析

摘　　要：目的探讨妊娠期肝内胆汁瘀积症(ICP)血脂变化及与母儿预后的关系。方法测定43例妊娠晚期ICP患者及40例正常孕妇肝功能、血脂及载脂蛋白,比较它们的变化及其对母婴的影响。结果ICP组总胆固醇、甘油三脂、低密度脂蛋白、载脂蛋白B显著高于正常孕妇组(P<0.05),高密度脂蛋白和载脂蛋白A明显低于正常孕妇组(P<0.05)。以95%正常孕妇血脂作为正常值范围,把ICP患者分为血脂正常和异常组,两组转氨酶、总胆红素、结合胆红素差异显著(P<0.05)。ICP病例血脂异常组的羊水污染发生率显著增高(P<0.05),产后出血、新生儿并发症、早产及围生儿死亡的发生增多。结论ICP患者血脂的异常可反映病情的严重程度,应作为ICP患者的重要监测指标。Objective To study the The eliange of serum lipids in intrabepatie eholestasis during gestational period and its relationship inthe prognosis . Methods 43 patients with ICP and 40 normal pregnant women in their late pregnancy were studied. The hepatic function, serum lipids and apelipoprotein were determined. The relationships between the serum lipid concentration and the abnormalities of hepatic funotion , complieations of mother and newbem in ICP were analyzed. Results The levels of CHOL ,TG,LDL and APOB of ICP group were dramatieally higber than these of the normal prenant group（ P 〈 0.05）. The levels of HDL and APOA were lower than of the normal pregnaney group（ P 〈 0.05 ）. When the serum lipid values of 95 % normal pregnant women were taken as the normal ranges and the ICP eases were divided into normal and abnormal serum lipid groups, the levels of ALT,AST, TB and DB in the abnormal serum lipid group were dramatically higher than thee in the normal serum lipid group（ P 〈 0.05） .The occurrence of meeonium stained amnionie fluid were dramatically increased （P 〈 0.05）, and the occurrence of postpartum bemorrhage, complications of newbem, premature delivery and perinatal death increased in the abnormal serum lipid group. Conclusions The abnormal change of serum lipids in ICP reflects the severity of ICP . So serum lipids should be examined routinely in ICP.
