闭合性肝损伤的早期诊断与治疗经验

摘　　要：作者15年来共收治71例闭合性肝损伤。全组病例均行手术治疗。死亡6例，死亡率为8．45％。本文就闭合性肝损伤的早期诊断和肝创面的处理进行讨论。着重介绍1987年10月以后大量动物实验取得成功的基础上应用医用粘合剂──α－氰基丙烯酸正丁酯对肝创面直接喷注，快速粘合修补止血的方法。认为采用该医用粘合剂操作简便，迅速粘合止血的效果满意，值得推荐。Seventy-one cases of closed liver trauma have been treated in recent 15 years. All were operated and of which 6 cases died with a mortality of 8.45%. This paper discuss the early diagnosis of the closed liver trauma and the tropical treatment of the injured liver surface. Our method, which has been introduced to treat 31 cases since Octber 1987, is based on many successful experiments. It employs a medicinal cementing agent-α-bubyl-cyanoacrylate, which may be sprayed Over the injured surface, so that a thin layer of the agent will be formed adhering to the wound immediately and the bleeding is stopped. This method is particularly helpful for deeply located wounds and in which poor exposure prevents it from suture repair. We found this new technique effective and convenient to use.
