摘 要:目的:探讨血必净注射液对脓毒症大鼠凝血功能及组织肿瘤坏死因子-α(TNF-α)的影响。方法:采用盲肠结扎穿孔术(CLP)制备脓毒症动物模型。96只健康Wistar大鼠按随机数字表法分为正常对照组、假手术组、模型组和血必净治疗组,后两组分别于CLP后0.5、12、24、36、48和60h经大鼠阴茎背静脉注射生理盐水或血必净注射液4ml/kg,分别于CLP后2、8、24、48和72h5个时间点各处死8只动物。各组大鼠取腹主动脉血检测血浆凝血酶原时间(PT)、活化部分凝血酶原时间(APTT)及纤维蛋白原(Fbg)含量;取肝、肺组织测定TNF-α蛋白水平。结果:CLP后2h模型组动物肝、肺组织TNF-α蛋白水平迅速升高(P均〈0.01),8h达到高峰,分别为正常对照组的2.57倍(P〈0.01)和4.77倍(P〈0.01)。血必净注射液治疗后可显著降低肝、肺组织TNF-α蛋白水平(P〈0.05或P〈0.01),于CLP后24h均恢复至伤前正常范围。模型组CLP后2~72h。PT、APTT有不同程度的缩短,血浆Fbg水平显著降低(P〈0.05或P〈0.01);血必净治疗组CLP后2~24hPT、APTT均较模型组明显延长(P〈0.05或P〈0.01),血浆Fbg含量回升。结论:血必净注射液可显著降低脓毒痒大鼠组织TNF-α蛋白水平,防止凝血功能异常,从而有效防止脓毒症的发生发展。Objective: To investigate the effects of traditional Chinese medicine Xuebijing injection (血必净注射液) on tissue tumor necrosis factor-α (TNF -α) expression and blood coagulation parameters in septic rats. Methods: Wistar rats were subjected to sepsis induced by cecal ligation and puncture (CLP). Ninety - six healthy rats were randomly divided into four groups: normal group, sham operation group, CLP model group, and Xuebijing - treated group. The two latter groups were given respectively intravenous injection of normal saline or Xuebijing injection with the dose of 4 ml/kg at 0.5, 12, 24, 36, 48 and 60 hours after the establishment of CLP model. Eight rats were sacrificed at 2, 8, 24, 48 and 72 hours post -CLP in the two latter groups. Prothrombin time (PT), activated partial thromboplastin time (APTT), and fibrinogen (Fbg) levels were determined. Tissue TNF -α protein levels in liver and lung tissues were also measured at various intervals. Results: TNF -α protein levels in liver and lung tissues were significantly increased at 2 hours after establishment of CLP model compared with those of the normal group (both P〈0.01), and they were 2.7:7 -fold (P〈0.01) and 4.77 -fold (P〈0. 01) of baseline values at 8 hours post -CLP, respectively. However, treatment with Xuebijing injection markedly reduced TNF-α protein levels both in liver and lung tissues (P〈0.05 or P〈0.01), recovering to normal baseline values at 24 hours post -CLP. In CLP model group, PT and APTT were significantly shortened, and plasma Fbg levels were significantly decreased at 2- 72 hours (P〈0.05 or p〈0. 01 ), and they were markedly improved in the Xuebijing- treated group at 2 - 24 hours (P 〈0.05 or P 〈 0.01 ). Conclusion : Xuebijing injection can inhibit tissue TNF-α protein synthesis and release in vital organs in rats with sepsis, thereby it may prevent the development of blood coagulation dysfunction and severe sepsis.
