摘  要:目的从分子水平鉴定丙型肝炎病毒(HCV)宫内感染的可能性,为确定HCV垂直传播提供更直接的依据。方法分别用免疫分析法和逆转录-聚合酶链反应检测婴儿和母亲的血清抗HCV和HCVRNA,对母婴HCVRNA阳性的血清作HCV包膜糖蛋白E2高可变区(E2HV)序列分析和比较。结果11例HCV感染的母亲所生的婴儿有6例生后脐血和静脉血抗HCV阳性,5例于1~5个月阴转,另1例持续阳性达13个月;5例于生后HCVRNA阳性,3例于1~6个月自然转阴,2例持续阳性分别达9个月和13个月;对2例持续HCVRNA阳性的婴儿和其母亲E2HVR序列分析发现,生后母婴间的氨基酸同源性高达93.33%,而2例母亲间同源性62.22%。结论丙型肝炎可以发生母婴传播,并可能为宫内感染。To explore the possibility of the intra-uterine infection of hepatitis C virus(HCV) at the molecuiar level and to provide a more direct evidence which identifies the existence of vertical transmission of HCV. Methods Anti-HCV and HCV RNA in sera from mothers and their babies were analysed by enzyme linked immunosobent assays and reverse transcription-polymerase chain reaction(RT-PCR) respectively. The hypervariable region of envelope gbcoprotein E2 in HCV RNA positive sera from two mother-infant palrs was sequenced and compared. Results Among 11 babies with their mothers infected with HCV,six were shown to be positive for anti HCV in both cord blood and venous blood at birth. Anti HCV disappeared in 5 babies after 1~5 months and persisted in only one baby for 13 months.HCV RNA was found in both cord blood and venous blood from 5 babies at birth. Among them, 3 bables got a serum conversion of HCV RNA after 1~6 months and 2 were constantly positive with HCV RNA for 9 months and 13 months respectively. The homologous rate of E2HV amino acid sequences between mother and her baby was 93.33%, 62.22%, Between two mothers conclusion This study indicate that HCV could be clearly transmitted from mother to infant, possibly occurring in utero.
