摘 要:目的 应用组织多普勒技术检测有家族史与无家族史高血压患者不同左心室构型舒张功能变化特点。方法 对92例有家族史高血压患者和97例无家族史高血压患者应用组织多普勒测定舒张早期速度(Em)、舒张晚期速度(Am),计算Em/Am的值。结果有家族史组Em、Em/Am值在4种左心室构型中均低于无家族史组。其中有家族史向心性重构型Em、Em/Am分别为(7.95±0.08)cm/s和(0.72±0.04),无家族史组为(8.77±0.58)cm/s和(0.80±0.08)。有家族史向心性肥厚型Em、Em/Am分别为(6.65±1.73)cm/s和(0.60±0.12),无家族史组向心性肥厚型为(7.80±2.30)cm/s和(0.70±0.10)。向心性重构型和向心性肥厚型Em、Em/Am值差异均有显著性(P〈0.05)。结论 家族史对左心室舒张功能具有一定影响,在向心性肥厚型中舒张功能受损最重。Objective To study the changes of left cardiac diastolic function in hypertensive patients with or without family history. Methods Ninty-two hypertensive patients with family history (FH + group ) and ninetyseven hypertensive patients without family history ( FH - group ) were studied. Early diastolic ( Em ) phase velocity, late diastolic (Am ) phase velocity and Ea/Aa ratio was calculated. Results In four remodeling patterns of left ventricular hypertrophy,Em and Em/Am values showed more decrease in FH + group than in FH - group. In concentric remodeling pattern,Em and Em/Am values of FH + group were (7.95±0. 08 ) cm/s and ( 0.72±0.04 ) respectively ,while those of FH - group were ( 8.77 ± 0.58 ) cm/s and (0.80 ± 0.08 ). In concentric left ventricular hypertrophy pattern, the values of FH + group were (6.65 ± 1.73 ) cm/s and 0.60 ± 0.12 vs( 7.80± 2.30 ) cm/s and 0.70 ± 0.10 of FH -group. Em and Em/Am value were significantly different between concentric remodeling pattern and hypertrophy pattern(P 〈 0.05 ). Condusion Hypertensive family history contributes to the diastolic function of left ventricular hypertrophy. The influence is significant in concentric left ventricular hypertrophy.
