氟比洛芬酯复合小剂量芬太尼用于术后静脉镇痛

摘　　要：目的探讨氟比洛芬酯复合小剂量芬太尼与单纯芬太尼术后静脉镇痛的临床效应。方法全髋置换术的患者48例,随机分为4组:A组,芬太尼1mg+0.9%NaCl共100mL;B组,芬太尼0.5mg+0.9%NaCl共100mL;C组,氟比洛芬酯50mg+芬太尼0.5mg+0.9%NaCl共100mL;D组,氟比洛芬酯50mg+芬太尼0.375mg+0.9%NaCl共100mL。参数设定:负荷量5mL,持续输注2mL/h,PCA2mL,锁定10min。观察24h内的镇痛评分(VAS),PCA使用次数及不良反应的发生情况。结果术后2h时B组的VAS显著高于A,C和D组(P<0.05),而2h后的VAS4组间差异无显著性(P>0.05)。24h内的PCA按压次数B组显著大于A,C和D组(P<0.05)。B,C和D组不良反应的发生率显著低于A组(P<0.05)。镇痛期间无呼吸抑制及异常出血等并发症发生。结论氟比洛芬酯复合芬太尼行术后静脉镇痛效果良好,可明显减少芬太尼的用量和不良反应的发生率。[objective] To compare analgesic efficacy and complications of intravenous postoperative analgesia with Flurbiprofen axeyil injection combined with a small dose of Fentanyl and with Fentanyl alone. [Methods] Forty-eight postoperative patients undergoing total hip arthroplasty were randomly divided into four groups with 12 cases each. Fentanyl 1 mg A group, Fentanyl 0.5 mg B group, Flurbiprofen Axeyil injection 50 mg plus Fentanyl 0.5 mg in group C and Flurbiprofen Axeyil injection 100 mg plus Fentanyl 0.375 mg in group D. The drugs in each group were diluted to 100 mL and infused by a pump in a rate of 2 mL/h with a patient-controlled bolus of 2 mL after a loading dose of 5 mL. The visual analog scale （VAS）, times of PCA and incidence of side effects were recorded during the period of postoperative 24 hours. [Results] The VAS of group B at 2 h after operation was significantly higher than that of group A, C and D （P 〈0.05）, which became similar 2 h later （P 〉0.05）. The accidences of nausea, vomiting, itching, somnolence in group A, C and D were significantly less than those in group A （P 〈0.05）. The demanding times for supplemental bolus in group B （13.3±1.4） was significantly higher than that of group A （6.6±1.4）, C （7.1±1.3） and D （6.9±1.4） （P 〈0.05）. No respiratory depression or abnormal bleeding occurred in four groups. [Conclusion] Intravenous postoperative analgesia with Flurbiprofen Axeyil injection combined with small dose of fentanyl has a better analgesic effect than Fentanyl alone and can reduce Fentanyl requirement and its side effects.
