COMPARISON OF THE PREVALENCE ESTIMATES OF OBESITY FOR PRESCHOOL CHILDREN DERIVED FROM NATIONAL AND INTERNATIONAL REFERENCE STANDARDS

摘  要:[目的]比较4种不同的参考标准对铜陵市学龄前儿童肥胖的评价结果,探讨不同标准的一致性。[方法]以铜陵市区12所幼儿园3~6岁2 184名学龄前儿童为对象,分别使用4种标准即全国9城市0~7岁儿童身高别体重标准(weight-for-height,1995年)(标准1)、全国8城市0~7岁儿童人体质量指数(body mass index,BMI)标准(1986年)(标准2)以及WHO推荐的0~6岁儿童身高别体重标准(weight-for-height,1975年)(标准3)和国际肥胖工作组(International Obesity Task Force,IOTF)推荐的2~18岁儿童青少年BMI标准(2000年)(标准4)评价肥胖率,利用综合评价(3个及3个以上标准同时判定为肥胖)的方法,计算4种标准的符合率、假阳性率和假阴性率。[结果]4种标准判定的肥胖率分别为8.5%、1.1%、2.8%、1.5%,差异有统计学意义(2χ=223.35,P=0.000),4种标准判定的同年龄性别儿童肥胖率的差异均有统计学意义;4种标准筛查肥胖的符合率分别为16.4%、100%、50.0%、96.9%,差异有统计学意义(2χ=126.92,P=0.000),各性别年龄组符合率的差别,仅6岁组差异无统计学意义(6岁组2χ=7.13,P=0.068);标准1、标准3和标准4的假阳性率分别为7.1%、1.4%和0,三者假阴性率均为0,标准2的假阳性率为0,而其假阴性率为19.4%,4种标准假阳性率和假阴性率的差异均有统计学意义(假阳性率2χ=326.75,P=0.000;假阴性率2χ=15.65,P=0.001)。[结论]4种标准筛查肥胖率差异显著,应谨慎使用。建议发展我国学龄前儿童肥胖评价标准并采用国际统一筛查标准。[Objectives] To compare the prevalence estimates of obesity for the preschool children in Tongling derived from 4 sets of reference values and to discuss the consistency among these reference values. [Methods] The prevalance of obesity of 2 184 urban children aged 3-6 years from 12 kindergartens in Tongling was assessed by 4 sets of reference values: weight-for-height reference values of urban children under 7-year (1995) (standard 1), reference data of body mass index (BMI) for children from birth to 7 of 8 cities in China (1986) ( standard 2) , WHO international reference data of the weight-for-height reference value for children from birth to 6 (1975) (standard 3), international cut off points for BMI for overweight and obesity by sex between 2 and 18 years recommended by International Obesity Task Force (IOTF) (2000) (standard 4) . The method of synthetic evaluation ("obesity" was defined when obesity were assessed by 3 or 4 standards simultaneously) was also employed to calculate the coincidence rate, false positive rate, and false negative rate. [ Results] The prevalanee of obesity in the children estimated by the 4 standards were 8.5%, 1.1%, 2.8%, 1.5%, respectively, and the differences were statistically significant (χ^2= 223.35, P = 0.000) . Moreover, the differences by sex-age-group were all statistically significant. The coincidence rate by the 4 standards were 16.4%, 100%, 50.0%, 96.9%, respectively, and it was found that the differences were statistically significant (χ^2 = 126.92, P = 0.000), and the differences among the coincidence rate by sex-age-group were statistically significant, but 6- year group was excluded (χ^= 7.13, P = 0.068) . False positive rates of standard 1, standard 3 and standard 4 were 7.1%, 1.4% and 0, respectively, and the false negative rate of each was 0.
