中心静脉导管引流加持续负压吸引治疗自发性气胸临床研究

摘　　要：目的 探讨在临床中应用改良后的中心静脉导管（前端造孔）引流加持续负压吸引治疗自发性气胸能否取得显著疗效。方法 40例自发性气胸患者随机分为两组，一组采用改良后的中心静脉导管引流加持续负压吸引（中心静脉导管组），另一组采用粗硅胶导管接水封瓶引流（粗硅胶导管组），比较两组意患者在平均肺复张时间、伤口感染率、胸痛发生率、胸积液发生率、皮下气肿发生率和导管堵塞发生率等方面有无显著差异。结果 中心静脉导管组平均肺复张时间明显少于粗硅胶导管组（P〈0．05），中心静脉导管组胸痛发生率、胸腔积液发生率、皮下气肿发生率均明显低于粗硅胶导管组（P〈0．05），两组伤口感染率、导管堵塞发生率无无显著差异（P〉0．05）。结论 应用改良后的中心静脉导管引流加持续负压吸引的方法治疗自发性气胸疗效显著、损伤性小、并发症少，是一项值得临床推广的好方法。Objective To observe the clinical effects of the improved central venous catheter combined with consecutive negative pressure suction in treating spontaneity pneumothorax. Methods 40 patients with spontaneity pneumothorax were randomly assigned to two groups： one group （central venous＇ s group） had the thoracic drainage of the central vein catheter combined with consecutive negative pressure suction, while the other group （thick silica gel catheter＇ s group） had the thoracic closed drainage of the thick silica gel catheter. Assay the occurence of the wound infection, the chest pain, pleural effusion, the pneumohypoderma and the catheter suffered up and the average time of alveolar recruitment of the two groups. Results As compared with those in the thick silica gel catheter＇ s group, the average time of alveolar recruitment were significantly less in central venous＇ s group （P〈0.05）, the occurrences of the chest pain, the pleural effusion and the pneumohypoderma were also less in central venous group （P〈0.05）. However, there were no difference between two groups in the occurrence of the wound infection and the catheter suffered up （P〉0.05）. Conclusion Improved central venous catheter combined with consecutive negative pressure suction showed significant effect, mini-invasiveness and low complication rate in treating spontaneity pneumothorax. And it is a good method and is worthy to popularizie clinically.
