60例慢性肾功能不全患者心肺损害临床分析

摘　　要：目的了解慢性肾功能不全（CRI）患者心、肺损害的临床表现和胸部X线片、心电图特点。方法回顾性分析2004年11月-2005年11月在吉林大学第一医院肾内科住院的60例CRI患者的临床资料及胸部X线片、心电图特点。结果CRI患者中高血压占88．3％，心功能不全占56．7％，心电图异常占71．7％。CRI患者胸部X线多有双下肺纹理增强、增多、紊乱、模糊，占86．7％；肺充血占6．7％；双下肺间质改变占6．7％；单侧或双侧胸腔积液占10．0％。结论CRI患者多伴有心、肺功能损害，早期透析治疗是防治心、肺损害的有效方法。Objective To study the clinical manifestation, chest X - ray and electrocardiogram features of heart and lung impairment in chronic renal insufficiency （CRI） patients, to investigate the causes involved. Methods Clinical data of 60 caseas with CRI were reviewed retrospectively which managed in our hospital between Nov. 2004 to Nov. 2005. Results The incidence of hypertension was 88.3%, cardiac insufficiency was 56.7% , abnormal electrocardiogram was 71.7%, abnormal chest X - ray appearance was 86.7%, lung congestion was 6. 7%, interstitial change of the lung was 67%, one or two sides of hydrothorax was 10. 0%. Conclusion The heart impairment of CRI patients allways has hypertension, anemia is higher than the incidence of lung impairment in CRI patients. The heart impairment is related to high blood pressure, anemia and acidosis. The lung impariment is ralated to uremic toxins, hypoproteinemia and cardia insufficiency. The treatment of early dialysis is effective in preventing and curing the heart and lung impairment.
