联合检测血清MMP-9、endostatin与VEGF水平对晚期NSCLC诊断和预后的意义

摘　　要：背景与目的MMP-9、endostatin和VEGF是与多种恶性肿瘤有关的重要的血管生长调控因子。本研究旨在分析非小细胞肺癌（NSCLC）患者MMP-9、endostatin、VEGF水平在临床及预后中的意义。方法应用酶联免疫吸附法（ELISA）检测92例晚期初治NSCLC患者血清MMP-9、endostatin及VEGF水平，良性对照组50例，健康对照组52例。结果①肺癌患者治疗前血清中MMP-9、endostatin及VEGF水平显著高于健康人（P〈0．001）及肺良性病患者（P〈0．001）。②以健康人检测值的x^-＋2s为临界值，MMP-9、endostatin及VEGF检测的敏感度分别为92．51％、88．32％和83．4％，特异度分别为79．1％、74．25％和75．63％。③肺癌患者血清MMP-9，endostatin水平以x^-＋s为临界值（565．61ng/ml，119．23ng/ml）分组，检测值高于临界组生存期较检测值低于临界值组为短，两组患者预后有显著差异（P〈0．05）。④部分患者化疗后检测三项指标，疗效达PR＋SD患者中，MMP-9水平下降组与上升组预后有显著差异（P〈0．05）。结论肺癌患者血清中MMP-9、endostatin及VEGF水平显著增高，具有较高的敏感性和较好的特异性，对于指导临床肺癌诊断、估计预后有着重要意义。Background and objective MMP-9,endostatin and VEGF are important angiogenic regulators of many kind of neoplasms. This study is to judge clinical and prognostic values of serum MMP-9,endostatin and VEGF levels in patients with non-small cell lung cancer （NSCLC）.Methods Serum levels of MMP-9,endostatin and VEGF were detected in 92 patients with NSCLC and 56 patients with benign pulmonary disease, compared with those of 52 eases in healthy group by ELISA method. Results ①The serum levels of MMP-9,endostatin and VEGF in pretreatment non-small cell lung cancer patients were significantly higher than those in patients with benign pulmonary diseases （P〈0.001） and healthy group（P〈0.001）. ②Serum MMP-9 levels reached a sensitivity of 92.51% and a specificity of 79.10% with a cutoff value of 117.17 ng/ml,whereas endostatin levels reached 88.32% sensitivity and 74.25% specificity at 100.31 ng/ml and VEGF levels reached 83.40% sensitivity and 75.63% specificity at 380.32 ng/ml. ③Serum MMP-9 and endostatin levels in pretreatment lung cancer patients were significant prognostic factors. ④The changes of serum MMP-9 level after chemotherapy was useful indicator of prognosis for these patients. Conclusion The serum levels of MMP-9,endostatin and VEGF are significant increased in NSCLC patients.They have high sensitivity and specificity,also they can be used as prognostic parameters in patients with NSCLC.
