高血压病合并频发室性期前收缩患者左心房局部功能的应变及应变率评价

摘　　要：目的探讨高血压病患者左心房功能的变化与频发室性期前收缩之间的关系。方法应用应变率成像技术对32例健康人、70例单纯高血压病患者及43例高血压病合并室性心律失常患者左房局部功能进行定量评价。结果与正常人相比,高血压病组的舒张早期应变率峰值差异具有统计学意义（P〈0.01）,高血压病合并频发室性期前收缩组的收缩期应变率峰值差异具有统计学意义（P〈0.05）;与单纯高血压病组相比,高血压病合并频发室性期前收缩组的收缩期应变率峰值差异具有统计学意义（P〈0.01）。结论高血压病合并频发室性期前收缩时,左房心肌储存器功能受损,左房管道功能下降;应变率指标能够可靠灵敏地反映高血压病合并频发室性期前收缩时左房功能的改变。Objective To explore the changes of left atrial functions in hypertensive patients with atrial arrhythmias, especially with frequent premature ventricular contractions. Methods Strain rate imaging（SRI） was performed on a total of 145 subjects,consisting of 32 controls,70 hypertension-alone, and 43 hypertension with frequent premature ventricular contractions, to explore the changes of left atrial functions. Results Early diastolic strain rate was significantly lower in hypertensive patients as compared with the control group（ P 〈0. 01）. Systolic strain rate decreased significantly in hypertensive patients with frequent premature ventricular contractions when compared with the control group （P 〈0. 05 ） or hypertension-alone（ P 〈0. 01）. Conclusions In hypertensive patients with frequent premature ventricular contractions, left atrial myocardial reservoir function is impaired, and conduit function decreases. Noninvasive quantification of left atrial function by SRI enables evaluation of left atrial dysfunction due to hypertension or frequent premature ventricular contractions in patients.
