摘 要:目的探讨高分辨力超声结合血管回声跟踪技术定量评价高血压病患者颈总动脉弹性参数的临床价值。方法原发性高血压患者60例,按血压水平分为1级高血压组、2级高血压组和3级高血压组,每组各20例;以健康志愿者20例为正常对照组。应用高频线阵探头测量双侧颈总动脉内-中膜厚度(IMT),采用血管回声跟踪技术获取双侧颈总动脉的弹性系数(Ep)、僵硬度(β)及顺应性(AC)三项指标,分析这些指标与血压的关系。结果在高血压病患者,Ep、β、颈总动脉最大内径(Ds)、颈总动脉最小内径(Dd)、IMT与收缩压、舒张压、脉压、平均动脉压显著正相关,而AC与收缩压、舒张压、脉压、平均动脉压负相关。颈总动脉IMT与Ep、β、Ds、Dd正相关,与AC显著负相关。结论随着血压的逐渐升高,颈总动脉IMT逐渐增厚而弹性逐渐减退;应用血管回声跟踪技术能够准确地定量评价颈总动脉弹性。Objective To investigate the elastic properties and intima-media thickness(IMT) of the common carotid arteries (CCA) as well as factors affecting these parameters in various grades of hypertension with high resolution ultrasound and echo-tracking technique. Methods Based on blood pressure levels,sixty primary hypertensive patients were divided into 3 groups(grade 1, grade 2 and grade 3) with 20 patients in each group. Twenty healthy volunteers were selected as controls. The carotid arteries were scanned bilaterally using a scanner equipped with a high frequency linear array probe. Systolic inner diameter(Ds), diastolic inner diameter(Dd) and IMT of the CCA were measured. Pressure-strain elastic modulus(Ep), stiffness (β) and the arterial compliance (AC) were obtained by means of echo-tracking system. The relations between these parameters and the blood pressure were analyzed. Results In patients with hypertension, Ep, β, Ds, Dd and IMT were correlated positively with systolic pressure, diastolic pressure, pulse pressure and mean arterial pressure while AC was correlated with these parameters negatively. No correlation was found between the heart rate and these parameters. IMT was correlated positively with Ep, β,Ds and Dd but negatively with AC. Conclusions With the increase of blood pressure, the elasticity of the CCA decreased while IMT increased. Echo-tracking technique may be a promising modality to assess CCA elastic properties accurately and quantitatively.
