摘  要:目的应用声学定量技术评价心房颤动(房颤)犬左心房功能改变,探讨钙激活蛋白酶系统与左房功能重构的关系。方法17只杂种犬随机分为房颤组(11只)和对照组(6只),通过心房快速起搏8周建立犬房颤模型,于起搏前后应用声学定量技术记录左心房容量-时间曲线,并计算相关指标。采用Western blot技术检测左心房肌钙依赖性中性蛋白酶(calpain)及其抑制剂钙蛋白酶抑制蛋白(calpastatin)的蛋白表达量。结果与对照组比较,房颤组左房储存器容积无明显改变;管道容积显著增加(P〈0.01);左房射血分数、峰值心房排空率(PAER)显著降低(P〈0.01);左房心室收缩末期容积、快速排空末期容积、心室舒张末期容积、心房排空容积增加(P〈0.01)。房颤组calpainⅠ、calpainⅡ蛋白表达明显高于对照组(P〈0.05),calpastatin蛋白表达明显低于对照组(P〈0.05)。calpainⅠ蛋白表达水平与左房PAER、左房排空分数、左室射血分数呈显著负相关(P〈0.05);calpastatin蛋白表达水平与左心房PAER、左房排空分数呈显著正相关(P〈0.05)。结论房颤时心房发生功能重构,表现为助力泵功能减低,管道功能增强,calpain系统激活可能是房颤时心房功能重构的重要分子机制。Objective To evaluate the atrial function in a canine atrial fibrillation model by acoustic quantification(AQ) and investigate the relationship between the calpain system and left atrial functional remodeling. Methods Seventeen dogs were randomly divided into 2 groups: normal control group(SR, n = 6) and atrial fibrillation group(AF, n = 11). AF were induced by rapid pacing for 8 weeks. AQ waveform of left atrial before and after 8 weeks of pacing were measured with AQ technique in these dogs. The protein expression of calpain Ⅰ ,calpain Ⅱ and calpastatin were assessed by Western blot. Results Left atrial reservoir volume had no changes after pacing; conduit volume incresed significantly in AF group compared with SR group( P〈0.01 ). In AF group, left atrial ejection fraction(LAEF), rapid emptying fraction(REF) and peak atrial emptying rate (PAER) decreased significantly ( P 〈 0.01 ) ; left atrial end-systolic volume, end-diastolic volume, end rapid emptying volume and active empting volume were greater than those in SR group ( P〈0.01 ). Amounts of calpain Ⅰ and calpain Ⅱ protein were significantly increased in AF group compared with SR group (P〈0.05); the expression of calpastatin protein was significantly decreased in AF group ( P〈0.05). The calpain Ⅰ protein levels were negatively correlated with left atrial PAER, REF and LAEF; the calpastatin protein level was positively correlated with left atrial PAER and REF ( P〈0.05). Conclusions Booster pump function of left atria impaires and conduit function increases during AF. The activation of calpain system may be an important molecular mechanism of atrial functional remodeling in AF.
