摘 要:目的 探讨正常肝脏MR灌注特征、量化指标和参考标准。方法 连续选取100例临床拟行上腹部MR检查、无肝脏病变的受检者行上腹部常规MR成像及MR肝灌注成像。选择包含门静脉主干、腹主动脉、肝实质的单层斜冠状层面、采用2DFSPGR序列行MR肝灌注扫描,平静呼吸,注射对比剂后连续扫描100s,每秒采集1幅图像,共获得100幅图像。在图像后处理工作站上获取腹腔于水平腹主动脉(abdominal aorta,AA)、门静脉主干(portal vein,PV)和肝实质的时间信号曲线(TIC),测量PV、肝实质的TIC峰值时间;以AA、PV的TIC为参照设置时间参数,分别得到肝实质肝动脉期、门静脉期的正增强函数(PEI),从而计算出肝实质的肝灌注系数(HPI)并对不同年龄组正常肝脏HPI进行统计分析。结果 100例正常肝脏门静脉主干的TIC峰值时间为(38.66±4.14)s,肝实质的TIC峰值时间为(56.24±4.47)s,HPI为(18.9±3.5)%。各年龄组(20~30岁、30~40岁、40~50岁、50~60岁)肝实质的HPI分别为(20.9±4.8)%、(18.1±3.7)%、(18.4±2.4)%、(19.1±3.6)%,各组均数之间差异无统计学意义(P〉0.05)。结论 本研究借助MR肝灌注成像技术,利用强化峰值时间及HPI,获得100例中国人正常肝脏灌注量化评估参考标准。Objective To study the blood supply characters of the normal livers by using MR perfusion imaging. Methods 100 adults (20 -60 years old) without liver diseases underwent conventional and MR perfusion imaging of the liver by using a single slice and an oblique coronal section 2D FSPGR sequence. Quiet breath, 100 s scan time after bolus injection of Gd - DTPA. 100 images of the same section were got. The time signal -intensity curves(TICs) of the abdominal aorta, the portal vein and the right liver lobe at the level of truncus celiacus were got in workstation and the peak time of TICs of portal vein and right liver lobe were measured. Postive enhancement integrals ( PEI ) of the parenchyma of the right liver lobe was measured in both artery and portal vein phase. Hepatic perfusion indexes(HPIs) of different age groups were calculated and analyzed. Results The peak time of the portal vein and liver parenchyma TICs of the 100 normal livers were (38.66 ±4.14) s and (56.24 ±4.47) s respectively . The HPIs of liver was (18.9 ±3.5)% . The HPI of liver were ( 20.9 ±4.8 )% , ( 18.1 ±3.7)% , (18.4±2.4)%,(19.1 ±3.6)% in group Ⅰ (20 -30 years old), Ⅱ (30 -40 years old), Ⅲ(40-50 years old) and Ⅳ ( 50 - 60 years old) respectively. The HPI difference between the four age groups showed no statisticaly significance( P 〉 0.05 ). Conclusion The reference criteria for quantifying the perfusion of normal livers by using peak time of TIC and HPI are got.
