摘 要:目的探讨强化教育对2型糖尿病并糖尿病足高危因素病人情绪及代谢的影响。方法60例有糖尿病足高危因素的2型糖尿病病人随机分为对照组和强化教育组,各组30例。健康教育前,用HAMILTON抑郁量表(HAMD)及HAMILTON焦虑量表(HAMA)评估病人情绪,检查空腹血糖(FBS)、糖化血红蛋白(HbAlc)、总胆固醇(TC)和甘油三酯(TG)。强化教育组给予强化教育,对照组给予传统的健康教育。教育第90天再用HAMD及HAMA评估病人情绪,教育第30天及第90天分别复查FBS、HbAlc、TC和TG。结果2组病人教育后抑郁和焦虑情绪均有改善,但强化教育组较对照组改善更明显(P〈0.01)。2组病人教育后FBS、HbAlc、TC和TG均显著降低(P〈0.05),但强化教育组较对照组降低更明显(P〈0.05)。结论强化教育优于传统的健康教育,它可以显著改善2型糖尿病并糖尿病足高危因素病人的抑郁、焦虑和代谢异常。Objective To investigate the effects of intensive education on mood and metabolism in patients with type 2 diabetes mellitus with a high risk of diabetic foot. Methods All 60 patients with type 2 diabetes mellitus and having a high dangerous factor of diabetic foot were randomly divided into the control group (30 cases) and the intensive education group (30 cases). The mood states of all the patients were estimated using HAMILTON Depression Scale (HAMD) and HAMILTON Anxiety Scale (HAMA). And fasting blood sugar (FBS), glycosylated hemoglobin (HbAlc), total cholesterol (TC) and triglyeride (TG) of all the patients were estimated before the conducting of the healthy education. Then the intensive education group was carried out the intensive education, and the control group carried out the conventional healthy education. HAMD and HAMA of all the patients were estimated again in the 90th day after the education. FBS, HbAlc, TC and TG of all the patients were estimated again in the 30th day and the 90th day after the education, respectively. Results The depression and anxiety of the two groups were significantly ameliorated after the education. And compared with the control group, those in the intensive education group were ameliorated more significantly after the education (P 〈 0.01). FBS, HbAlc, TC and TG of the two groups were lowered significantly after the education (P 〈 0.05). But, compared with the control group, those indexes of the intensive education group were lowered more significantly after the education (P 〈 0.05). Conclusions The intensive education is better than the conventional health education. The intensive education can significantly improve the depression, anxiety and metabolic abnormality in the patients with Type 2 diabetes mellitus and having a high dangerous factor of diabetic foot,
