腹腔镜筋膜内子宫切除术后宫颈囊肿的发生及诊疗初探

摘　　要：目的:探讨腹腔镜筋膜内子宫切除术(CISH)后宫颈囊肿的发生原因和治疗。方法:改进腹腔镜筋膜内子宫切除手术方法,对宫口松弛者弃用常规管口较小的锯齿管刀,改用相应口径的锯齿管刀,并注意切下的宫颈组织是否完整(为中心有孔的环状),如缺如,则电凝残存宫颈内膜组织。将改进前后的病例分为甲乙两组进行比较。结果:甲组(改进前)出现12例宫颈囊肿,乙组(改进后)未再发生宫颈囊肿;宫颈囊肿的发生与切下的宫颈完整性有关。结论:宫颈囊肿的发生是由于CISH术中残留宫颈腺体所致,因此改进CISH手术方法有利于预防宫颈囊肿的发生。Objective： To probe into the genesis and the diagnosis and treatment of the naboth cyst after the CISH. Methods： To improve the intrafascial SEMM hysterectomy （CISH） operation, routine serrature sword was changed to corresponding antilinear sword in the palace chalasia patient and pay attention if the cervix tissue is complete or not, if it is absence, then the survival endocervix tissue was electric coagulation. Compare two groups about the case of before and after of the improve device of the CISH. Results： There is 12 cases naboth cyst in the preimproved group, naboth cyst don＇t occur in the improved group. It showed the occurrence of naboth cyst is related to the completation of cervix. Conclusion： The occurrence of cervix cyst is caused by residual cervix gland after CISH. To improve the refined degree of the CISH operation can prevent the happen of the naboth cyst.
