摘 要:目的探讨乙型肝炎病毒感染的免疫学标志(HBVM)与乙型肝炎病毒脱氧核糖核酸(HBVDNA)的相关性。方法采用酶联免疫吸附试验(ELISA)法和荧光定量聚合酶链反应(FO-PCR)技术分别对1573例乙型肝炎(下称乙肝)患者血清进行HBVM(HBsAg、抗-HBs、HBeAg、抗-HBe、抗-HBc)及HBVDNA检测。结果HBsAg(+)、HBeAg(+)、抗-HBc(+)模式,HBVDNA≥10^3copy/ml检测率高达91.84%,其余HBsAg(+)、抗-HBe(+)、抗-HBc(+)为58.13%,抗-HBe(+)、抗-HBc(+)模式为33.33%,抗-HBs(+)、抗-HBe(4-)、抗-HBc(+)为16.28%,抗~HBs(+)模式为10.53%及HBVM全部阴性模式10.00%,并对HBVM各种模式HBVDNA表达的频率分布进行分析,结果显示HBsAg(+)、HBeAg(+)、抗-HBc(+)模式,HBVDNA拷贝值由低(≥10^3 copy/ml)向高(10^8 eopy/ml),且50.20%乙肝患者在10^7~10^8copy/ml范围内。HBVM其余模式,多数患者集中在较低拷贝值范围内(10^3copy/ml)。分别是HBsAg(+)、抗-HBe(+)、抗-HBc(+)为71.4%,抗-HBs(+)、抗-HBe(+)、抗-HBe(+)为42.86%,抗-HBe(+)、抗-HBe(+)为66.70%。结论血清HBVM阳性患者中72.38%血清HBVDNA≥10^3eopy/ml,且HBsAg(+)、HBeAg(+)、抗-HBe(+)模式的血清中HBVDNA≥10^3 copy/ml检出率及HBVDNA的拷贝值均明显大于其他各种模式(P〈0.05),病毒复制活跃,传染力强,但其余模式仍可能存在病毒复制。Objective To investigate the relationship between HBV DNA expression and serum HBV markers. Methods FQ-PCR technique and ELISA method were applied to detect HBV DNA expression and serum HBV markers in 1 573 patients with hepatitis B. Results The amount of HBV DNA was different in 3 HBV immunological markers modes. The amount of HBV DNA expression was higher in HBV surface antigen, HBV e antigen and anti-HBcAg antibody positive mode than that of other modes, with 91.84% of positive rate. The positive rate of HBV DNA in other modes was 58. 13% (HBV surface antigen, anti-HBeAg antibody and anti-HBcAg antibody positive mode), 33.33% (anti-HBeAg antibody and anti-HBcAg antibody positive mode), 16.28% (anti-HBsAg antibody, anti-HBeAg antibody and anti-HBcAg antibody positive mode), 10.53% (only anti-HBsAg antibody positive mode) and 10.00% (HBV markers negative mode), respectively. Conclusion Serum HBV DNA was above 10^3 copies/ml in 72.38% of HBVM positive patients, and serum HBV DNA positive rate and HBV DNA copies was highest in HBV surface antigen, HBV e antigen and anti-HBcAg antibody positive mode, with active viral replication and strong infective power. Viral replication might still exist in other modes.
