吸切器下鼻甲成形术和下鼻甲低温等离子射频消融术的疗效比较

摘　　要：目的比较吸切器下鼻甲成形术和下鼻甲低温等离子射频消融术治疗肥厚性鼻炎的长期疗效。方法 62例慢性肥厚性鼻炎患者中,32例行吸切器下鼻甲成形术(microdebrider-assisted trubinoplasty,MAT 组),30例行下鼻甲低温等离子射频消融术(coblation,CAT 组)。根据视觉模拟评分法(visual analogue scal,VAS)分别对两组术前和术后3,6和12个月时的症状改善以及并发症随访评估、评定两种手术的长期疗效。结果两组手术后3,6和12个月时的症状 VAS 评分均较术前差异有统计学意义;12个月时 CAT 组改善程度轻度降低,MAT 组则持续有效;尽管 MAT 组手术出血显著多于 CAT 组,两组手术时间、痂皮持续时间以及涕倒流却差异无统计学意义。结论吸切器下鼻甲成形术和下鼻甲低温等离子消融术均是治疗肥厚性鼻炎的良好方法,但前者长期疗效更佳。Purpose To compare the postoperative outcome between microdebrider-assisted turbinoplasty and eoblation.Methods Sixty two patients that had nasal obstruction and hypertrophied turbinate mueosa were collected. Thirty-two patients were treated with microdebrider-assisted turbinoplasty（group MAT ） and thirty patients with eoblation （group CAT）. Postopertive changes of nasal obstruction were evaluated by Visual Analogue Scale at 3, 6 and 12 months after treatment. Duration of surgery, postoperatiev crust, postoperative bleeding and postnasal drip were also compared between the two groups.Results Nasal obstruction was improved significantly in both groups at 3,6 and 12 months after the procedure. However,when comparison was made between the two goups,symptoms improvement was more evident in group MAT than CAT group at 12 months after the surgery. There were no significant differences in degree of postnasal drip,operation time,or duration of crust formation between the two groups. Conclusion Microdebrider-assisted turbinoplasty and coblation are both effective in treatment of hypertrophic inferior tubinate,however,microdebrider-assisted turbinoplasty seems to be a better one regarding long term effect.
