摘  要:心肌中Ⅰ亚型心肌肌钙蛋白是一种能快速诊断急性心肌梗塞的特异蛋白质,同时它也是小面积心梗及不稳定心梗患者的灵敏早期诊断物质.肌红蛋白长期以来也被人们认为是心梗的早期诊断物质,但其缺点是缺乏足够的特异性.血清肌酸激酶亦为诊断心梗的物质,这是由于它对心肌梗塞特异性强,在心梗发生后2-8h内在血中浓度急剧上升.目前美国已将上述3种物质取长补短结合研制成一种全新的灵敏诊断试剂,是目前全球最简捷快速诊断早期急性心梗的可靠产品.Cardiac Troponin Ⅰ(cTnⅠ)is a 100% cardiac specific protein which can be used to diagnosis Myocardial Inforetion(MI). The eTnI is also a very sensitive marker making it possible to detect minor myocardial damage in unstable angina. Myoglobin has long been recognized as the earliest biochemical marker for the diagnosis of MI. Serum myoglobin peak at approximately 6 - 9 h, but is cleared from circulation rapidly and returns to normal within 24 - 36 h. Myoglobin is also abundant in both cardiac and skeletal muscle. Therefore, it lacks adequate specificity. Thus, CK - MB is a very good marker for MI, because of its excellent specificity and it rises in serum within 2 - 8 h of onset of acute myocardial infarction. However, it may raise from the injury of skeletal muscle. A combination test kit of Troponin Ⅰ, Myoglobin and CK - MB provides an early diagnosis of MI and is the most important technology for patiem for a high chance of survival.
