摘  要:目的探讨α-adducin基因Gly460Trp与盐敏感性高血压及其肾损害、血浆肾素活性的关系。方法收集中国汉族EH组150例,正常血压(normaltensive,NT)对照组150例。采用改良的Sullivan's法将150例高血压病患者分为盐敏感性(SS)和非盐敏感性(NSS)高血压病组,采用聚合酶链反应-限制性片段长度多肽性分析检测α-adducin基因型,采用放射免疫法测定PRA、血尿β_2- MG及尿ALB。结果(1)α-adducin基因TrpTrp和Trp等位基因频率在高血压组显著高于对照组(P<0.05);在SS和NSS组:α-adducin TrpTrp型基因频率显著高于NSS组(P<0.05)。在EH和SS组,PRA水平与α-adducin各基因型间未见有显著变化(P>0.05);(2)SS组晨尿β_2-MG及尿ALB/Cr显著高于NSS组(P<0.05),血β_2-MG在SS和NSS组无明显差异;在SS组中α-adducin TrpTrp基因型组,尿β_2-MG及尿ALB/Cr显著高于GlyTrp、GlyGly基因型组(P<0.05)。结论(1)α-adducin Gly460Trp TrpTrp型基因与盐敏感性高血压有显著相关,与PRA水平无相关性;(2)α-adducin TrpTrp型基因与盐敏感性高血压早期肾损害相关,可能是盐敏感性高血压肾损害的独立危险因素。Objective To investigate the association between the α-adducin Gly460Trp and saltsensitive hypertension and renal injury in Chinese people. Methods The study was performed in 150 hypertensive and 150 normal tensive(NT) subjects in china, 150 patients with essential hypertension were divided into salt-sensitive(SS) and non-salt-sensitive(NSS) groups by modified Sullivan' s method, The Genotypes of α-adducin gene were determined by Restriction Fragment Length Polymorphism-Polymerase Chain Reaction ( PCR- RFLP) ; Using radio-immunity to test levels of PRA,blood and urine β2-Mg and urine ALB in 150 essential hypertension subjects. Results ( 1 ) A higher prevalence of α- adducin TrpTrp and Trp allele among patients with essentialhy pretension was noted (P 〈 0.05 ). In SS and NSS groups, there were statistical significance of α-adducin TrpTrp genotype between SS and NSS subjects (P 〈0.05 ). but there were no statistical significance between the level of PRA and a- adducin genotype ( P 〉 0.05 ) ; (2) The levels of urine β2- MG and ALB/Cr in SS patients were higher than NSS patients, there was statistical significance(P 〈 0.05 ), but there were no statistical significance of the level of blood β2-Mg ( P 〉 0.05 ) . In SS group, The levels of urine β2-MG and ALB/Cr in group of a-adducin TrpTrp genotype were higher than α-adducin GlyTrp and GlyGly genotype, there was statistical significance ( P 〈 0.05 ). Conclusions ( 1 ) The α-adducin TrpTrp genotype is significantly associated with SS hypertension;(2 ) α-adducin TrpTrp genotyp is significantly associated with SS hypertension accomping renal ingury.
