外周血管病变与非心肌梗死患者冠状动脉病变的临床研究

摘　　要：目的探讨外周动脉病变对非心肌梗死冠心病(Coronary heart diease,CHD)多支病变的诊断价值。方法对60例非急性心肌梗死的可疑冠心病患者行冠状动脉造影(CAG),其中明确冠心病43例(冠心病组),正常17例(正常组),对于冠状动脉病变依据修正的Genisini评分法进行评分。利用超声检测外周动脉内膜中膜厚度(inti-ma-media thickness IMT)、斑块等指标,并进行统计学分析。结果(1)不同评分的冠心病组患者与正常组3组外周动脉IMT间差异均有显著性意义(P<0.05);(2)外周动脉IMT与冠脉积分的对数值间(修正Genisini评分)呈正相关(P<0.05);(3)外周动脉IMT、斑块与冠状动脉多支病变均呈正相关(P<0.05)。结论外周动脉粥样硬化的斑块对冠状动脉多血管病变的预测价值较高,可以作为观察冠状动脉多支病变的重要指标。联合评价颈动脉和股动脉粥样硬化可为预测冠心病提供更多信息。Objective To investigate the diagnosis value of peripheral atherosclerosis to non - AMI patient＇s multiple coronary pathological changes. Methods Giving 60 cases patients CAG who were skeptical coronary heart disease in non - acute myocardium infarct, among them coronary heart diseases 43 cases, normal 17 cases, The modified Genisini score system is used to evaluate the coronary artery, Using ultrasound to check the indexs of plaque and periphery artery IMT meanwhile give statistics analyse. Results （ 1 ） The incidence of atherosclerosis plaque and IMT in the two groups have significance （ P 〈 0. 05 ）. （2） The periphery artery IMT are positive correlation with log （revised Gensini＇s grade） （ P 〈 0. 05 ）. （3） The periphery artery IMT and pluque are positive correlation with the multiple coronary pathological changes. Conclusion The atherosclerosis pluque of peripheral artery has higher sensitiveness in predicting the multiple coronary pathological changes. It can be taken as the important index to observe muhiple coronary pathological changes. The association evaluation of carotid and femoral artery atherosclerosis can be provided more informations for predicting the coronary heart disease.
