摘　　要：目的：总结右侧胸壁3孔胸腔镜下心脏不停跳房间隔缺损修补手术的疗效.方法：采用股动脉、静脉插管建立体外循环,术中不阻闭升主动脉,体外循环不降温或浅低温（32±1）℃,全胸腔镜下行房间隔缺损修补手术.结果：本组无死亡,术后右侧气胸1例;手术时间1.8～2.7 h,体外循环时间36～72 min,术后呼吸机辅助时间2～4 h,术后胸液引流量明显减少,术后住院时间5～8 d.结论：右侧胸壁3孔胸腔镜下心脏不停跳房间隔缺损修补手术创伤小,恢复快,安全可靠.Objective： To summarize the experience of the closer of atrial septal defect through three ports under total thoracoscopicy and beating heart. Methods： The operations were performed through three right anterior ports under total thoracoscopy and beating heart using femorofemoral and femoral-right atrial extracorporeal circulation. The aorta was not cross-clamped without the extracorporeal circulation hypothermia. The repair of the defect was performed successfully. Result： There was no operative death and no morbidity directly related to the three portotomy approach. The time of operation was 1.8-2.7 h; the time of extracorporeal circulation lasted 36-72 min and ventilation 2-4 h; the volume of drainage decreased and time for hospitalization was 5-8 days. Conclusion： The closer of atrial septal defect through three ports under total thoracoscopicy and beating heart is a safe and effective technique with less drainage and shortened hospital stay.
