复方鬼箭羽汤对高血压病胰岛素抵抗和血液流变学的影响

摘　　要：目的:观察复方鬼箭羽汤的降压和改善高血压病胰岛素抵抗及血液流变学的作用。方法:将符合纳入标准的80例高血压病患者,随机分为治疗组(42例)和对照组(38例),治疗组予口服复方鬼箭羽汤加卡托普利,对照组服用卡托普利,两组疗程均为6周,观察治疗前后两组空腹血糖(FPG)、空腹血清胰岛素(FINS)、胰岛素敏感指数(ISI)和血压、血液流变学的变化。结果:治疗后,两组患者与其治疗前比较不仅血压明显下降(P<0.01),而且FINS明显降低(P<0.05或P<0.01),ISI显著升高(P<0.05或P<0.01),对照组治疗前后血液流变学的变化均有显著性差异(P<0.05),治疗组治疗前后血液流变学的变化均有非常显著性差异(P<0.01);两组治疗后比较,治疗组FINS显著低于对照组(P<0.05),ISI显著高于对照组(P<0.01);两组血压下降幅度比较无显著性差异(P>0.05);而治疗组与对照组治疗后的血液流变学的比较亦有显著性差异(P<0.05)。结论:复方鬼箭羽汤对高血压病胰岛素抵抗有明确的改善作用,且能改善高血压病血液流变学异常。Objective： To observe the hypertension-reducing function of Compound Guijianyu Decoction and the improvement of insulin inhibition of hypertension and blood rheology. Methods： 80 cases of patients with hypertension were randomized into the treated group （42 cases） and the control group （38 cases）. The treated group were treated by Compound Guijianyu Decoction and Katuopurui, and the control group by Katuopurui. The course of treatment for both groups was 6 weeks. The change of FPG, FINS, ISI, blood pressure and blood rheology in two weeks was observed before and after the treatment.Results：After the treatment, Patients of two groups had an obvious decrease of blood pressure （P 〈 0.01） and FINS （P〈 0.05 or P 〈 0.01 ） and an remarkable increase of ISI （P 〈 0.05 or P 〈 0. 01 ）. The change of blood rheology of the control group before and after the treatment had significartt difference （P〈 0.05）, while the change of blood theology of the treated group had much mare significant difference （P〈0.01）. FINS of the treated group was remarkably lower than that of the control group （P〈0.05） while ISI higher than that of the control group （P〈0.01）. Two groups had no significant difference （P〉0.05） in the deerease range of their blood pressure. Two groups had significant difference （P〈0.05） in their blood rheology after the treatment. Conclusion： Compound Guijianyu Decoction can improve insulin inhibition and the abnomality of blood rheology of hypertension.
