摘 要:目的探讨全身炎症反应综合征(SIRS)患者血清载脂蛋白AⅠ(ApoAl)、C-反应蛋白(CRP)和白蛋白(ALB)水平的变化特点及对预后的影响。方法入组当天检测ApoAⅠ、CRP及ALB水平,比较49例SIRS患者预后与上述指标的关系。结果49例SIRS组患者的ApoAⅠ(0.84±0.361)g/L及ALB(27.58±7.25)g/L水平低于非SIRS组患者(P〈0.05或P〈0.01)而CRP水平(54.35±18.85)mg/L及SAPSⅡ评分(23.5±9.74)高于非SIRS组患者(P〈0.01)。随着病情的变化,SIRS组中死亡患者的ApoAI(0.65±0.31)g/L及ALB(25.31±6.88)g/L水平进一步降低,并低于存活组,而CRP(68.54±28.94)mg/L水平及MODS评分(11.2±3.8)高于存活组(P〈0.05或P〈0.01)。结论ApoAⅠ、ALB和CRP三种急性时相蛋白水平的变化有助于SIRS病情危重程度及预后的判断。Objective To study the characteristic change of apoporotein A Ⅰ (ApoA Ⅰ ), C-reactive protein (CRP), albumin (ALB) in patients with SIRS and to investigate the effects on prognosis. Method The serum levels of ApoA Ⅰ , CRP and ALB were measured at the admission day. The relationship between prognosis and the three above markers were analyzed in 49 patients with SIRS. Results The serum level of ApoA Ⅰ (0.84 ± 0.361) g/L and ALB (27.58 ± 7.25) g/L in 49 SIRS patients were lower than those in non-SIRS group (P 〈 0.05 or P 〈0.01) and the level of CRP (54.35 ± 18.85) mg/L and SAPS Ⅱ scores (23.5 ± 9.74) of SIRS patients were higher than those in non-SIRS. The levels of ApoA Ⅰ (0.65 ± 0.31 ) g/L and ALB (25.31 ± 6.88) g/L in dead patients of SIRS group further decreased as the patients' condition deteriorated and were lower than in patients who survived , but CRP (68.54 ± 28.94) mg/L and the MODS scores (11.2 ± 3.8) in dead patients were higher than those in patients who survived, (P 〈 0.05 or P 〈 0.01 ). Conclusions The measurement of serum ApoA Ⅰ , CRP and ALB is helpful for predicting the prognosis of the patients with SIRS. The elevation of serum CRP and the reduction in serum ApoA Ⅰ and ALB may predict serious and bad prognosis of SIRS.
